Instructions In submitting a proposal, the Candidate accepts in full and without restriction the special and general conditions including annexes governing this Contract as the sole basis of this procedure, whatever his own conditions of services may be, which the Candidate hereby waives. The Candidates are expected to examine carefully and comply with all instructions, forms, contract provisions and specifications contained in this Request for Proposal. Scope of services Project Information & Geographical Area The Contractor (Insurance Provider) shall provide comprehensive insurance services for the Fraternity Associations main office building and related assets under a Property All Risks Insurance Policy. The insured property consists of a four (4) floor building used for educational and recreational purposes, including kindergarten, school, and activity setups supporting children and youth programmes. The total insured reinstatement value of the building and its related assets is USD 500,000. The Associations facilities include: - Kindergarten, school, and recreational setups supporting educational and recreational activities and any other activities related to the insured operations; - One (1) elevator; - Two (2) backup generators (60 KVA and 25 KVA); - Office furniture, equipments, fixtures, and related contents. The scope of insurance services shall include, but not be limited to: 1. Property All Risks coverage for the building and its contents, covering fire, explosion, natural disasters, theft, malicious damage, water damage, impact, and other accidental physical loss or damage unless specifically excluded in the policy. 2. Third-Party Liability coverage for bodily injury and/or property damage occurring within the insured premises. 3. Specific Third-Party Liability coverage for up to forty-five (45) registered students aged between eight (8) and thirteen (15) years attending educational and recreational activities at the premises, with clearly defined liability limits per occurrence and in aggregate. 4. Issuance and delivery of all insurance policy documents, certificates, and endorsements required under this Contract. 5. Claims handling and support, ensuring timely processing and fair settlement of covered losses. shall apply to the Fraternity Association premises located at: Beirut, Old Airport Road, Amleye Entrance, facing Ahmad pharmacy The Contractor shall ensure that all services are provided in accordance with applicable laws, regulations, and industry standards, and in compliance with the FABO Code of Conduct Cost of proposal The Candidate shall bear all costs associated with the preparation and submission of his proposal and the Fraternity Association is not responsible or liable for these costs, regardless of the conduct or outcome of the process. Eligibility and qualification requirements This call for proposal is open for participation of any legal/registered entity / individual in Lebanon. Entities / individual who are not registered in Lebanon welling to participate and in case if award will be subject to the Non-residential tax (8.5% deduction from the original awarded contract value). Candidates are not eligible to participate in this procedure if they are in one of the situations listed in article 33 of the General Terms and Conditions for Service Contracts Ver3 2020. Candidates shall in the Proposal Submission Form attest that they meet the above eligibility criteria. If required by the Contracting Authority, the Candidate whose proposal is accepted shall further provide evidence satisfactory to the Fraternity Association of its eligibility. As a rule, the timely arrival of a proposal with the Fraternity Association is the Candidates responsibility. Irrespective of the reason, proposals arriving after the deadline for the submission of proposals, will be considered late and thus rejected. Candidates are also requested to certify that they comply with the Code of Conduct for Contractors. Exclusion from award of contracts Contracts may not be awarded to Candidates who, during this procedure: are subject to conflict of interest are guilty of misrepresentation in supplying the information required by the Fraternity Association as a condition of participation in the Contract procedure or fail to supply this information Documents comprising the Request for Proposal The Candidate shall complete and submit the following documents under the submission part in this this proposal: (Annex 1) Terms of Reference - Sign and Stamp the document (Annex 2) Proposal Submission Form - Duly completed and signed by the Candidate (Annex 3) Organisation and Methodology - Duly completed and signed by the Candidate using the structure in (Annex 3) Annex 4 - General Terms and Conditions for Service Contracts - Ver3 2020 - Sign and Stamp the document Annex 5 - Code of Conduct for Contractors - Sign and Stamp the document Portfolio. highlighting the Candidates experience in the specific field of the requested Service in the Lebanon/Middle East . The proposal and all correspondence and documents related to the Request for Proposal exchanged by the Candidate and the Fraternity Association must be written in the language of the procedure, which is English. A.6. Financial Proposal (Insurance for Building) The Financial Proposal shall be presented as a lump-sum annual amount in USD using the Proposal Submission Form in Annex 3. The remuneration of the Insurance Provider under the Contract shall be determined as follows: Lump-Sum / Annual Insurance Premium: The Insurance Provider shall indicate in its proposal the total annual insurance premium in USD for providing Property All Risks Insurance coverage for the Fraternity Associations main office building located in Beirut, Lebanon. Coverage of fixed installations and facilities including one (1) elevator and two (2) backup generators (60 KVA and 25 KVA). Third-Party Liability coverage for bodily injury and/or property damage occurring within the insured premises. Specific Third-Party Liability coverage for up to forty-five (45) registered students aged between eight (8) and thirteen (15) years attending educational and recreational activities at the premises. The insurance coverage period shall be clearly stated and shall be for one (1) year. The Insurance Provider shall clearly indicate: The annual premium amount; Deductibles applicable per claim; Coverage limits per occurrence and in aggregate; Any exclusions; Any applicable taxes or VAT (to be shown separately). Any costs or charges not explicitly mentioned in the Contract shall be deemed included in the insurance premium and no additional payments shall be made. For unregistered suppliers, an 8.5% withholding tax shall be deducted from the total payable amount in accordance with applicable regulations. Validity The Proposal shall remain valid and open for acceptance for ninety (90) days after the closing date for submission of proposals Submission of proposal and closing date Proposals must be submitted by email to the address indicated on the front page of this Request for Proposal and must be received no later than the closing date and time specified. The Proposal shall include all required annexes and supporting documents as specified in this Request for Proposal.
