1. The Subject Of The Order Are Specialized Care Services For People With Mental Disorders In Their Places Of Residence, Who Are The Wards Of The Municipal Social Welfare Center In Chełmno (Hereinafter Also Referred To As Wards).2. The Detailed Scope Of Services Constituting The Subject Of The Order Is Specified In The Regulation Of The Minister Of Social Policy Of September 22, 2005 On Specialized Care Services (Journal Of Laws Of 2024, Item 816), Hereinafter Referred To As The Regulation On Specialized Care Services, And Results From Administrative Decisions Issued By The Ordering Party For Individual Clients Under The Act Of March 12, 2004 On Social Assistance (Journal Of Laws Of 2025, Item 1214, As Amended). 3. The Planned Number Of People Covered By Specialist Care Services Is 8 People, Including Seven Children Aged Up To 18 And One Adult. 4. Specialized Care Services For People With Mental Disorders Include: 1) For Children And Adolescents: A) Behavioral Therapy; B) Neuro-Pedadic Therapy; C) Sensory Integration Therapy; D) Physical Rehabilitation2) For Adults: A) Teaching And Developing Skills Necessary For Independent Living; B) Care - As Support For The Treatment Process: C) Physical Rehabilitation And Improvement Of Disturbed Body Functions To The Extent Not Covered By The Provisions Of The Act Of August 27 2004 On Health Care Services Financed From Public Funds (Journal Of Laws Of 2024, Item 146;D) Housing Assistance. 5. Estimated Number Of Hours Of Providing Specialist Care Services: 1) 640 Hours In The Basic Scope (Average Monthly 160 Hours);2) 1,100 Hours In The Scope Covered By The Option.6. One Hour Of Providing Services Should Be Understood As A Clock Hour (60 Minutes), Counted As The Actual Time Of Providing Services In The Place Of Residence Of The Client. It Does Not Include The Travel Time Or Arrival Time Of The Person Providing The Service To The Recipients Place Of Residence And The Return Time.
