1. The Subject Of The Proceedings Is The Successive Deliveries Of Bread And Confectionery Products (Also Called Assortment) To The Clinical Hospital. Dr. J. Babiński Spzoz In Krakow, Based On Partial (Daily) Orders Of The Ordering Party, In The Assortment Consistent With Annex No. 1.1 To The Swz, Which Is Also The Price Form.2. The Subject Of The Order Will Be Implemented In Accordance With The Requirements Of The Ordering Party Specified In The Document Entitled: Description Of The Subject Of The Order. Bread And Confectionery, Constituting An Annex To The Swz And In Accordance With The Provisions Of The Contract - Annex No. 4 To The Swz. Including, As Part Of The Deliveries, The Ordering Party Requires, Among Others: A) The Delivered Assortment Will Be First Class, Fresh, Cut, Packaged And Marked In Accordance With The Applicable Standard. Delivery And Collection According To Net Weight. B) Continuous Deliveries, 6 Times A Week From Monday To Saturday, Maximum Until 12:00, With Delivery And Payment By The Contractor And Quantitative And Qualitative Verification In The Presence Of The Contractor. Delivery On Saturday Is To Provide Bread For Sunday And Monday. C) The Assortment Will Be Delivered By The Contractor Directly To Hospital Wards And To The Ordering Partys Food Warehouse In The Kitchen Building (18 Collection Points), In Types And Quantities Consistent With The Partial (Daily) Demand Of The Ordering Party. D) An Authorized Representative Of The Ordering Party Will Report The Daily Demand Each Time By Phone Or E-Mail, 1-2 Days In Advance (Working Days) Before The Required Date Deliveries.E) The Label Of Each Product Should Include, Among Others, The Production Date And The Expiration Date. The Ordering Party Absolutely Requires That On The Day Of Delivery Of The Product To The Ordering Party, Its Shelf Life Is At Least 3/4 Of The Entire Shelf Life. F) In The Event Of Delivery Of A Smaller Quantity Of The Product Than Provided For In The Contract, The Contractors Remuneration Will Be Adjusted Based On The Actually Delivered Quantity Of The Subject Of The Order.3. Deliveries Of The Assortment Will Be Made By The Contractors Own Transport, At His Expense And Risk.
