Part 1: Purchase And Delivery Of Medical Equipment - Description Of The Subject Of The Order Can Be Found In Annex No. 1A To The Swz, The Scope Of The Order Includes: 1. Electronic And/Or Manual Blood Pressure Measuring Device In Various Sizes, Including A Set Of Cuffs For Children 52. Device For Quick Diagnostics - Various Tests, Including Crp 13. Ecg Device 14. Bilirubinometer 15. Dermatoscope 36. Doppler Flow Detector 17. E-Stethoscopes 38. Phantom For Learning Breast Self-Examination 19. Pressure Holter 210. Holter Ecg 111. Oxygen Concentrator 112. Bactericidal And Virucidal Uv Lamp 313. Refrigerator With Temperature Monitoring 114. Medical Furniture - Doctors Chairs 415. Air Purifier 216. Otoscope 217. Pulse Oximeter 1018. Spirometer 119. Stethoscope (Ordinary, Internal Medicine, Pediatric) 220. Infant Examination Table 121. Scale For Disabled People 122. Wheelchair For A Child 123. Wheelchair For An Adult 13. Provisions Common To The First Part Of The Order: 3.1. The Ordering Party Requires That The Delivered Order Constituting The Subject Of The Order Be Brand New, Unused, Have The Required Technical Parameters Specified In Annex 1A To The Swz (Detailed Description Of The Subject Of The Order), Be In An Undamaged, Technically Functional, Complete And Ready For Use And Met The Standards Required By Polish Law And Was Free From Physical And Legal Defects. The Ordering Party Excludes The Delivery Of Post-Exhibition Equipment. 3.2. The Ordering Party Requires That The Offered Subject Of The Order, Which Is Classified As Medical Devices, Meets The Requirements Imposed By Law Regarding Admission To Trading In The Territory Of The Republic Of Poland And Also Has The Registration Certificates, Certificates And Approvals Required By Law - In Accordance With The Act Of April 7, 2022 On Medical Devices (Journal Of Laws Of 2024, Item 1620) And Also Had The Appropriate Marking And The Ce Declaration Of Conformity.3.3. Before The Planned Delivery, The Contractor Will Inform The Ordering Party About It, Agreeing On The Date And Expected Time Of Delivery Of The Subject Of The Order. 3.4. Delivery Of The Subject Of The Order Will Take Place On Working Days, I.E. Monday - Friday, At Hours Agreed With The Person Delegated To Contact The Contractor. 3.5. The Scope Of The Order Includes In Particular The Purchase Of The Subject Of The Order Specified In Annex No. 1A To The Swz, Together With Its Delivery, Unloading, Bringing In And Assembling Or Installing The Equipment That Requires It In The Places Indicated By The Ordering Party. Assembly/Installation Works Should Be Performed In Accordance With The Recommendations Of Device Manufacturers. 3.6. Devices Cannot Be Refurbished, Demonstration Or Prototype Devices And Must Represent A Brand New Model. The Subject Of The Order Must Be Unrefurbished, Complete, And Manufactured No Earlier Than June 2025.
