1. The Subject Of The Order Is The Provision Of Sanitary Transport Services - Type S Specialist Rescue Team For The Municipal Hospital In Ruda Śląska Sp. Z O. O. A Detailed Description Of The Subject Of The Order In The Scope Of The Provision Of Services Was Specified In The Swz, In Annex No. 1 To The Swz/Agreement And In Annex No. 7 To The Swz.2. Pursuant To Art. 95 Section 1 Of The Act, The Ordering Party Requires That Persons Performing Activities Directly Related To The Performance Of The Subject Of The Order (I.E. Persons Coordinating The Work Of Rescue Teams) Be Employed Under An Employment Contract.3. The Requirements Regarding The Method Of Documenting The Employment Of A Person Under An Employment Contract, The Ordering Partys Control Rights And Sanctions For Failure To Meet The Requirements Referred To In Point 2 Above, Have Been Specified In Detail In The Draft Provisions Of The Contract, Which Will Be Introduced Into The Content Of The Procurement Contract (Annex No. 7 To The Swz).
