Detailed Description Of The Subject Of The Order: 1. The Subject Of The Order Are The Services Of A Personal Assistant For A Person With A Disability For Persons With Disabilities - Program Participants Who Are Residents Of The Bartoszyce Rural Commune, To The Extent Adequate To The Individual Needs Of A Person With A Disability Resulting From The Scope Of Activities Card As Part Of The Personal Assistance Service For The Personal Assistant For A Person With A Disability Program For Local Government Units - Edition 2026.2. The Direct Implementer On The Part Of The Ordering Party Is The Municipal Social Welfare Center In Bartoszyce.3. The Services Will Be Provided Under The Personal Assistant For People With Disabilities Program For Local Government Units - 2026 Edition From The Solidarity Fund - A State Earmarked Fund.4. The Scope Of Services Provided By A Personal Assistant To A Person With A Disability Will Include Assistance In Performing Everyday Activities And Support In Functioning In Social Life.5. The Care Provided Must Be Consistent With The Provisions Of The Program Entitled: Personal Assistant For A Person With A Disability For Local Government Units - 2026 Edition, The Terms Of The Procedure, The Offer Submitted By The Contractor, This Contract And Generally Applicable Legal Provisions.6. The Ordering Party Plans To Outsource Personal Assistance Services To A Non-Governmental Organization Referred To In Art. 3 Section 2 Of The Act Of April 24, 2003 On Public Benefit Activities And Volunteering Or The Entity Referred To In Art. 3 Section 3 Of This Act.7. Estimated - Expected Number Of People Covered By The Services Of A Personal Assistant For A Person With A Disability: 1) 6 Disabled Adults Requiring A High Level Of Support, Having A Certificate Of A Significant Degree Of Disability; 2) 3 Adults With Multiple Disabilities; 3) 2 Children From The Age Of 2 To 16 Years Of Age Who Have A Certificate Of Disability Together With The Indications In Points 7 And 8 In The Certificate Of Disability - The Need For Permanent Or Long-Term Care Or Assistance From Another Person Due To The Significantly Limited Ability To Live Independently And The Need For Constant Daily Participation Of The Childs Guardian In The Process Of Treatment, Rehabilitation And Education.8. Program Participants Will Be Supported In Performing Everyday Activities And Functioning In Social Life.9. The Provision Of The Service That Is The Subject Of The Order Will Take Place In The Amount Of Not More Than 6111 Hours Per Year And Not Less Than 20% Of The Indicated Maximum Number Of Hours During The Duration Of The Contract.10. The Estimated And Expected Number Of Hours Is: 1) A Total Of 3,231 Hours (Clock)
