Tenders Are Invited For Providing The Services Of An Independence Trainer In A Training Apartment At Ul. Stefana Kardynała Wyszyńskiego 16A/9 In Białogard

The Subject Of The Order Is Carried Out As Part Of The Project Entitled Development Of Social Services In Białogard Project Number Fepz.06.19-Ip.01-011/25 Co-Financed By The European Social Fund Plus Under The European Funds For Western Pomerania Program 2021-2027. Ii. Subject Of The Order1. The Subject Of The Order Is The Provision Of An Independence Trainer Service For Adults Using A Training Apartment, At Risk Of Social Exclusion Or Affected By It, In Particular: 1) People In The Crisis Of Homelessness, 2) People Leaving Institutions, Hospitals Or Other Forms Of Institutional Care, 3) People Requiring Support In The Process Of Becoming Independent.2. The Service Will Be Provided In A Training Apartment Run By The Ordering Party And - In Justified Cases - In The Participants Local Environment.3. The Ordering Party Declares That It Has A Training Apartment Equipped In Accordance With Applicable Law, Including The Regulation Of The Competent Minister On Training And Supported Apartments, Ensuring Conditions For The Provision Of The Service.4. The Aim Of The Service Is To Prepare The Participant To Function Independently In The Local Environment After Leaving The Training Apartment. Iii. Dimension And Period Of Service Implementation1. The Service Will Be Provided 3 Hours A Day, 7 Days A Week. 2. The Period Of Support Implementation For One Participant Is Approximately 6 Months.3. The Ordering Party Assumes That During The Project Implementation Period Of 23 Months, The Independence Process Will Cover A Total Of At Least 4 People.4. The Number Of Hours And The Number Of Participants May Change Depending On The Individual Needs Of Participants And Project Assumptions, While Maintaining The Maximum Value Of The Contract. Iv. Target Group1. The Participants Of The Service Will Be People Qualified For The Project By The Municipal Social Welfare Center In Białogard.2. Support Will Be Provided Based On An Individual Diagnosis Of The Participants Life And Social Situation. V. Scope Of Activities Of An Independence Trainer1. The Contractors Tasks Include In Particular: 1) Diagnosis And Support Planninga) Conducting A Functional Diagnosis Of The Participant,B) Developing An Individual Path To Self-Empowerment (Iśu), Consistent With The Project Objectives,C) Periodic Updating Of The Iśu In Cooperation With A Social Worker Of Mops.2) Training In Everyday Living Skillsa) Learning How To Run A Household,B) Planning The Household Budget And Rational Management Of Funds,C) Organizing The Day And Free Time,D) Shaping Health-Promoting Habits And Hygiene.3) Social Competence Traininga) Developing Communication And Social Skills,B) Strengthening Decision-Making Independence And Responsibility,C) Learning How To Solve Problems And Coping With Difficult Situations.D) Maintaining Family Ties;E) Solving Interpersonal Conflicts And Developing Decision-Making Autonomy;F) Participating In The Life Of The Local Community4) Socio-Professional Activationa) Motivating To Ta
