The Subject Of The Order Is: 1. Delivery Of Tools For Surgical Procedures In Hybrid Technology To The Headquarters Of The Ordering Party (Kraków, Os. Złotej Jesieni 1), In Accordance With The Description And Requirements Contained In Annex No. 1A To These Specifications. • The Validity Period/Warranty Period Of The Subject Of The Order Cannot Be Shorter Than 12 Months From The Date Of Delivery. • The Contractor Undertakes To Conduct, At The Request Of The Ordering Party, Training Of The Ordering Partys Staff At Its Registered Office - Os. Złota Jesieni 1, Kraków With The First Delivery Of The Goods To The Szpitalna Pharmacy On The Date Agreed In Writing With The Ordering Party, But No Later Than 30 Days From The Date Of Conclusion Of The Contract. • The Contractor Will Be Obliged To Submit The Attendance List From The Training, With Signatures Of Trained Users Confirming Their Actual Presence, To The Hospital Pharmacy Within 5 Business Days From The Date Of The Training. • The Ordering Party Reserves The Right To Ask Contractors, At The Stage Of Examination And Evaluation Of Offers, To Provide Samples Of The Offered Subject Of The Contract In Order To Test And Check Them. The Costs Associated With This Are Borne By The Contractor. • Offers That Do Not Include The Full Scope Of The Subject Of The Contract Will Be Rejected. • The Remaining Terms Of The Order Are Specified In The Draft Contract Constituting Annex No. 3A To The Swz. 2. Rental Of Hybrid Generators - 2 Pieces (Apparatus), In Accordance With The Description And Requirements Contained In Annex No. 1B To The Swz. • The Limit Parameters Relating To The Apparatus Are Described In Annex No. 1B To This Tor. • Additional Conditions Relating To The Subject Of The Order: A. The Contractor Will Provide The Ordering Party With A Warranty For The Duration Of The Apparatus Rental, For A Period Of Min. 24 Months From The Date Of Signing The Contract In Question; B. The Contractor Will Bear All Costs Related To The Maintenance Of The Leased Item (E.G. Technical Inspections In Accordance With The Manufacturers Recommendations, Ongoing Repairs And Maintenance) During The Duration Of The Contract. C. The Costs Of Periodic Inspections, Servicing And Maintenance Of The Device, Including Consumable Materials, Are Covered By The Contractor, D. If It Is Necessary To Replace Consumable Parts During The Technical Inspection, The Cost Of These Parts Is Borne By The Contractor - After Each Inspection, The Contractor Ensures The Issuance Of A Certificate Of Operation Of The Device, E. The Contractor Will Provide Training To Employees Of The Medical Equipment Department, General And Oncological Surgery Department In A Number And Time That Ensures Correct And Safe Operation Of The Devices Within 2 Business Days From The Date Of Delivery Of The Device. The Cost Of Disposable Materials Used During The Training Will Be Covered By The Contractor. • The Remaining Terms Of The Order Are Specified In The Draft Contract Constituting Annex No. 3B To The Swz.
