Recreational, Cultural And Sporting Services – Provision Of Sports And Recreation Services In The Form Of Subscriptions For Bgk Employees And Their Families And Accompanying Persons.. 1. The Subject Of The Order Is The Provision Of Sports And Recreation Services On The Basis Of Subscriptions Enabling The Use Of Sports And Recreation Services Of Various Nature For Authorized Users, I.E. Employees Of Bank Gospodarstwa Krajowego, Their Children And Accompanying Persons. 2. Sports And Recreation Services (Hereinafter: Service) Will Be Provided As Part Of Monthly Subscriptions (Hereinafter: Subscription) Enabling The Use Of Sports And Recreation Services In Sports Or Sports And Recreation Facilities In Poland. 3. By Sports And Recreation Facility (Hereinafter: Facility), The Ordering Party Means A Building Or An Independent Premises In Which Sports And Recreation Services Covered By The Order Are Provided. Sports Activities And/Or Recreational Services Offered In One Facility (At One Administrative Address) By The Same Entity Will Be Treated As One Facility. 5. The Ordering Party Appropriately Divided The Subject Of The Order Into A Basic Order And An Order Covered By Option Rights. 1) The Basic Order Includes: 1. Provision Of Sports And Recreation Services On The Basis Of Subscriptions Enabling The Use Of Sports And Recreation Services Of Various Nature For Authorized Users, I.E. Employees Of Bank Gospodarstwa Krajowego, Their Children And Accompanying Persons. 2. The Scope Of Sports And Recreational Services Includes At Least: Swimming Pool, Gymnastics, Yoga, Gym, Aerobics, Martial Arts Lessons, Dance Lessons, Pilates, Fitness, Aqua Aerobics For The Ordering Partys Employees And Accompanying Persons. For Children Of Employees Up To 18 Years Of Age, The Scope Of Services Includes, Depending On The Selected Form Of Service, At Least: Swimming Pool (Only Swimming Pool Facilities) Or: Swimming Pool, Martial Arts And Dance Lessons, Gym. 3. The Subscription Should Entitle The User To Access Sports And Recreational Facilities And Activities Without The Need To Incur Additional Costs, Except For Access To Facilities And Activities Indicated By The Contractor As Paid Or Partially Paid. 4. The Contractor Undertakes To Provide, Under The Appropriate Subscriptions, Access To The Services For Authorized Users, I.E.: A) Employees - Persons Employed By The Ordering Party Under: Employment Contracts And Contracts For The Provision Of Management Services, B) Accompanying Persons Registered By The Employees, C) The Employees Children, I.E. The Employees Own Children And Children Adopted Or Accepted For Upbringing Within A Foster Family And Children Of The Spouse Or Partner Up To The Age Of 18 In The Scope Of: I. Exclusively Using Designated Swimming Pool Facilities, Ii. Sports And Recreation Subscriptions - Designated Swimming Pool Facilities, Martial Arts And Dance Lessons, As Well As Fitness Classes And Access To The Gym - In Accordance With The Requirements Of The Facilities Regarding Their Use By Persons Under 18 Years Of Age. 5. As Part Of The Subscription, The Contractor Will Provide The Employee With The Following Options: A) An Open Service, I.E. Unlimited Subscriptions, B) Limited Subscriptions. 6. Through Unlimited Subscriptions Of A Nature
