The Subject Of The Order Is A Service Consisting In The Provision Of Specialized Care Services Under The Act Of March 12, 2004 On Social Assistance, The Act Of March 12, 2004 On Social Assistance (Consolidated Text: Journal Of Laws Of 2025, Item 1214, As Amended) And The Regulation Of The Minister Of Social Policy Of September 22, 2005 On Specialized Care Services Regulation Of The Minister Of Social Policy Of September 22, 2005 On Specialized Care Services (Consolidated Text: Journal Of Laws Of 2024, Item 816) Ensuring Access To Rehabilitation, Revalidation And Educational Activities For Children And Adolescents With Mental Disorders, In Exceptional Cases, If They Are Unable To Obtain Access To The Classes Referred To In Art. 7 Of The Act Of August 19, 1994 On Mental Health Protection (Consolidated Text: Journal Of Laws Of 2024, Item 917) In Their Place Of Residence In Accordance With Medical Recommendations Or Other Indications Justifying The Specific Needs Of The Centers Residents. The Type Of Services, The Scope Of Services, The Period Of Provision Of Services, And The Number Of Hours Will Be Determined Individually For Each Child In The Administrative Decision, Which Constitutes The Basis For The Provision Of Services For Each Specialist. In Order To Ensure The Highest Quality Of Services And Taking Into Account The Well-Being Of Each Child, It Is Recommended That The Maximum Number Of Services Be Provided To Each Child By 1 Specialist. Subject Of The Order, Consisting In The Provision Of Services: Speech Therapist The Ordering Party Reserves The Obligation For The Contractor To Personally Perform Key Parts Of The Order Consisting In The Provision Of Specialized Services: Speech Therapist.1.2 The Tasks Of The Person Conducting Individual Classes Within The Scope Of The Subject Of The Order Will Include: 1) Conducting Classes In The Field Of Specialized Care Services For Disabled Children With Mental Disorders In Accordance With Their Qualifications; 2) Adapting The Content Of Classes And The Forms And Methods Of Work To The Individual Needs And Capabilities Of Children Resulting From, Among Others, Types Of Disorders And Disabilities; 3) Keeping Documentation Of Classes Carried Out, Including The Development Of: A. Assessment Of The Childs Social Functioning, B. Preparing A Problem Diagnosis, C. Defining The Goals Of The Services Provided, D. Defining An Individual Action Plan, E. Periodically Assessing The Effects Of Specialized Services, F. Correcting The Action Plan; 4) Implementing The Developed Individual Action Plan With The Child; 5) Assessing The Childs Progress And Analyzing The Effectiveness Of The Help Provided To The Child And Making Adjustments To The Action Plan, According To The Childs Needs; 6) Cooperation With Other Specialists Working With The Child In Order To Increase The Effectiveness Of Therapy; 7) Providing Instructions, Advice And Counseling, And Conducting Consultations With Parents/Guardians In The Field Of Working
