Jhpiego Corporation, an affiliate of Johns Hopkins University, is an international non-profit, nongovernmental organization that works to improve the health of women and families throughout the world. Jhpiego Nigeria is in the process of engaging the services of a reputable and licensed Health Management Organization (HMO). The vendors are encouraged to read through the EOI carefully to understand the requirements before submission of the proposal. 1.1 REQUIRED SERVICES. Provision of comprehensive health insurance coverage for all staff members of Jhpiego Nigeria. Estimate total number of staff is 150 to 250, which is broken down as follows: i. Individuals without family are estimated at 30% ii. Individuals with family estimated at 70% Other terms related to the services: The quote should include a list of the states covered by the HMO List of the hospitals in each of the states covered Vendors are required to give the best quote for the various available options/plans for medical insurance coverage for the staff profile detailed above. Detailed information regarding each option/plan should be provided 2.1. Quotation Should Include the Following: A breakdown of the financial offer, indicating the unit cost, total cost as well as other applicable costs, and schedule of the premium where necessary. All offers should be in Nigeria naira, valid for the contract period 4.1 ELIGIBILITY All applicants should provide the following: Certificate of registration/incorporation, memorandum and articles of association, and valid Tax Identification Number (TIN). Vendor must be licensed/authorize to provide HMO services for (please provide a copy of supporting documentation) Company profile (briefly reflecting the core competencies, and key management personnel and clearly stating the physical verifiable office address, mobile number, and e-mail address). Proof of or license to offer required services as given by the appropriate regulatory body (if applicable). Minimum of three (3) reputable references active within the last three years from clients/organizations Note that applicable government taxes will be deducted before making payments for services provided 5.1 Proposal Submission Jhpiego will not accept any proposal submitted in any other way.
