Tenders Are Invited For Providing Comprehensive Sterilization Services For The Municipal Hospital In Rabka - Zdrój Sp. Z O. O.

1. The Subject Of The Order Is The Paid Provision By The Contractor To The Ordering Party Of Comprehensive Sterilization Services Consisting Of: - Washing, - Disinfection, - Inspection Of Tools For Wear And Damage, - Packing In Paper And Foil Bags/Sleeves Or Crepe Paper, - Sterilization Of Tools, Medical Instruments, Medical Equipment (Hereinafter Referred To As Equipment) Specified In The Assortment And Price Form And Delivered By The Ordering Party After Preliminary Disinfection In A Hard, Tightly Closed Container. After The Sterilization Service Is Completed, The Ordering Party Will Collect The Entrusted Equipment In Disinfected Clean Transport Packaging. 2. The Ordering Party Allows And Provides For The Division Of The Order Into Two Parts (Tasks), I.E.: Task No. 1: Urgent Sterilization (Cito) The Detailed Scope Of The Subject Of The Order For Individual Tasks Is Specified In The Assortment And Price Form Constituting Annex No. 2 To The Swz. 3. Services Described In Point 1 The Contractor Will Provide Services From Monday To Saturday From 7.00 A.M. To 7.00 P.M. In The Contractors Central Sterilization Room (Hereinafter Referred To As Cs). The Submission Of Equipment For Sterilization Will Be Each Time Confirmed By An Order Card Signed By The Representatives Of The Parties. If The Contractor Detects Wear Or Damage To The Instrument Upon Admission To The Cs, He Or She Will Inform The Equipment User Directly About This Fact By Phone And/Or Enter A Note In The Sterile Material Issue Card From The Cs. 4. The Performance Of The Sterilization Service And The Release Of The Entrusted Equipment Will Be Confirmed By A Release Card Signed By The Representatives Of The Parties. The Contractor Bears Full Responsibility For The Medical Equipment Submitted For Sterilization Until It Is Returned To The Ordering Party. 5. The Contractor Guarantees The Sterility Period Specified On The Packaging, Provided That The Ordering Party Complies With Appropriate Storage Conditions For Sterile Equipment. 6. A Detailed Description Of The Comprehensive Sterilization Services Provided By The Contractor To The Ordering Party Was Specified In The Price And Product Range Form Constituting Annex No. 2 To The Swz. This Description Should Be Read Together With Any Changes To The Content Of The Swz, E.G. As A Result Of Responses To The Contractors Inquiries. The Quantities Given In Annex No. 2 To The Swz Are The Estimated Demand During The Duration Of The Order Used To Calculate The Offer Price, I.E. Determine The Maximum Remuneration Of The Contractor. The Use Of The Services Will Depend On The Actual Needs Of The Ordering Party. The Ordering Party Reserves The Right To Change The Scope Of The Subject Of The Order By Reducing It, And The Ordering Party Guarantees The Implementation Of The Subject Of The Order At A Level Not Lower Than 50% Of The Size Of The Subject Of The Order.
