Tenders Are Invited For Health Services – Provision Of Medical Services For Employees Of The Łukasiewicz Research Network - Institute Of Aviation And Their Family Members (Other Persons Covered By The Packages)
Health Services – Provision Of Medical Services For Employees Of The Łukasiewicz Research Network - Institute Of Aviation And Their Family Members (Other Persons Covered By The Packages). 1. The Subject Of The Order Is The Provision Of Medical Services For Employees Of The Łukasiewicz Research Network - Institute Of Aviation And Their Family Members (Other Persons Covered By The Packages), Hereinafter Referred To As Authorized Persons Or Authorized Persons. 2. Family Members (Other Persons Included In The Packages) Are Understood By The Ordering Party As Spouses, Partners, Children Up To 26 Years Of Age, Children Over 26 Years Of Age, Parents-In-Law, Parents. 3. The Number Of Packages Specified In The Paragraph Above Is The Forecast Quantity Provided For The Purpose Of Calculating The Offer Price By The Contractors. The Contractor Will Receive Remuneration For The Actual Number Of Packages Purchased, But Not Less Than 70% Of The Contract Value. The Contractor Is Not Entitled To Any Claim, Including For Lost Profits, If The Number Of Employees, Family Members And Individual Packages Is Not Achieved In Relation To The Forecast Numbers. 4. During The Term Of The Contract, Changes May Occur In The Number Of People Covered By The Service / Number Of Packages. If An Employee Or Other Eligible Person Resigns From The Service, The Ordering Party Is Not Obliged To Pay Contributions For Them From The Next Day Of The Month After The Month In Which The Entitled Person Resigns From Using The Service. 5. The Contractor Will Ensure The Availability Of At Least 500 Facilities Providing Services Specified In The Description Of The Subject Of The Order (Hereinafter Referred To As Opz) Throughout Poland, Including In The Capital City Of Warsaw (Administrative Boundaries) At Least 20 Facilities, In The Tricity, Rzeszów And Wrocław (Administrative Boundaries) At Least 3 Facilities In Each Location, Provided That Within The Administrative Boundaries Of Voivodeship Cities The Contractor Will Ensure The Possibility Of Using The Full Range Of Services Specified In The Packages Referred To In The Description Of The Subject Of The Contract. 6. The Ordering Party Intends To Exercise The Option Right In Accordance With The Rules Specified In The Order Documents.
