Tenders Are Invited For Delivery Of A Universal Ultrasound Device, A Pressure Holter And An Ecg Holter As Part Of The Grant Project No. Fenx.06.01-Ip.03-0001/23 Support For Primary Health Care (Poz).
Part 1: Delivery Of A Comprehensive Ultrasound Device. The Subject Of The Order Is The Delivery And Commissioning Of A Comprehensive Ultrasound Device - 1 Piece. The Contractor Is Obliged To Provide The Ordering Party With An Operating, Maintenance And Servicing Manual Prepared In Polish, As Well As Warranty Documents (Cards) And Other Necessary Documents (E.G. Technical Passports, Certificates) - Together With The Delivery Of The Device. The Offered Subject Of The Order Must Be Brand New, Manufactured No Later Than In 2024, Not Post-Exhibition, Not Reconditioned, Not Being A Demo Device - Complete And Ready For Use Without The Need To Incur Any Additional Investment Purchases. The Offered Equipment Must Have The Required Certificates And Markings Provided For In The Provisions Of The Act On Medical Devices, Proving The Required Authorization For Use And Marketing In Poland, I.E. The Ce Certificate And Declaration Of Conformity In Accordance With The Regulation Of The Minister Of Health On Essential Requirements And Procedures For Assessing The Conformity Of Medical Devices. In Addition, The Offered Equipment Must Be A Medical Device Approved For Trading And Use In Poland In Accordance With The Provisions Of The Act Of April 7, 2022 On Medical Devices. For The Offered Set: The Contractor Will Provide At Least 24-Month Warranty Period. The Scope Of Delivery Also Includes Training Of The Ordering Partys Employees In The Proper Operation Of The Device, Within The Deadline Agreed With The Ordering Party. Swz Part Ii (Opz) - List Of Required Technical And Operational Parameters For Part 1, In Which The Ordering Party Specified The Minimum Required Technical And Operational Parameters For The Equipment For The Above-Mentioned. Parts. If The Swz Indicates A Trademark, Patent Or Origin Of The Subject Of The Order, The Ordering Party Allows Equivalent Equipment - Equivalence Means The Possibility For The Contractor To Offer Products With Different Trademarks, Patents Or Origins, But With The Same Properties Required By The Ordering Party. The Contractor Is Obliged To Complete The Order On The Terms And Conditions Described In The Draft Provisions Of The Contract Constituting The Swz Part Iii - Ppu - Respectively For Part 1 Of The Executed Order. The Ordering Party Does Not Reserve The Obligation For The Contractor To Personally Perform Key Tasks. The Ordering Party Does Not Stipulate That Only Sheltered Workshops And Other Contractors Whose Activities, Or The Activities Of Their Organizationally Separated Units That Will Perform The Contract, May Apply For The Contract Include The Social And Professional Integration Of People Who Are Members Of Socially Marginalized Groups, In Particular Those Indicated In Art. 94 Section 1 Points 1 - 10 Of The Public Procurement Law. The Ordering Party Does Not Envisage The Possibility Of Providing The Existing Contractor With Additional Supplies Referred To In Art. 214 Section 1 Point 8 Of The Public Procurement Law.
