The Subject Of The Order Is The Service Called Providing Rehabilitation/Physiotherapy Services For Residents Of The Zagnańsk Commune As Part Of The Project Entitled: Cus In Zagnańsk - New Opportunities For Residents!.1) Terms Of Contract Implementation:A) The Services Of A Rehabilitator/Physiotherapist Will Be Provided In The Place Of Residence Of The People Covered By The Service.B) The Service Contractor Provides All Rehabilitation Equipment, Preparations And Personal Protective Equipment Necessary To Perform The Service.C) Physiotherapists/Rehabilitators Providing Services In The Place Of Residence Must Have Appropriate Qualifications, I.E. A Specialist In The Field Of Physiotherapy Or A Physiotherapist With A Masters Degree Referred To In Art. 4 Section 4 Points 1 And 2 Of The Act Of September 25, 2015 On The Profession Of Physiotherapist (Consolidated Text: Journal Of Laws Of 2023, Item 1213), Or A Physiotherapist With A Diploma, Certificate Or Other Document Referred To In Art. 13 Section 1 Point 5 Letter B And C Of This Act, With At Least 2 Years Of Experience As A Physiotherapist, A Valid License To Practice As A Physiotherapist And An Entry In The National Register Of Physiotherapists. D) The Hour Of Providing The Service Is A Clock Hour, I.E.: 60 Minutes. E) Number Of Hours Of Services To Be Performed During The Contract Period: 4,536 F) Estimated Number Of Participants: 42 People. G) Place Of Task Implementation: Zagnańsk Commune. H) Recipients Services Will Be Directed By The Client To Residents Of The Zagnańsk Commune With: - A Mild, Moderate Or Significant Disability Or Having A Certificate Equivalent To The Above-Mentioned. (In Accordance With Article 5 And Article 62 Of The Act Of 27 August 1997 On Vocational And Social Rehabilitation And Employment Of Disabled Persons (Journal Of Laws Of 2024, Item 44); - Children With Disabilities Up To 16 Years Of Age With A Disability Certificate; - Persons With A Medical Certificate Confirming The Need To Perform Rehabilitation/Physiotherapy Treatments (Hereinafter Referred To As Participants) Who Will Be Covered By The Task The Service Of A Rehabilitator/Physiotherapist (Hereinafter Referred To As The Service). The Price Indicated In The Offer Form For The Services Covered By This Order Must Include All Costs Related To The Execution Of The Order.
