The Subject Of The Order Is The Implementation Of The Task Entitled: Renovation Of An In-House Road With Renovation Of The Water And Sewage Connection In The Area Of Marr Business Park Nad Drwiną 10, Including The Following Works: A) Renovation Of A Section Of The Road With A Base For Heavy Vehicles, B) Renovation Of A Section Of The Road With A Base For Passenger Cars, C) Renovation Of The Water Supply Connection, D) Renovation Of The Sewage Connection, E) Renovation Of A Section Of The Road Made Of Cobblestones Road.2. A Detailed Description Of The Subject Of The Order Is Included In Annex No. 1 To Swz.3. Requirements Related To The Implementation Of The Order In Terms Of Employment By The Contractor Or Subcontractor On The Basis Of An Employment Relationship Of Persons Performing The Activities Specified By The Ordering Party In The Scope Of The Implementation Of The Order, If The Performance Of These Activities Involves Performing Work In The Manner Specified In Art. 22 § 1 Of The Act Of June 26, 1974 - Labor Code (Journal Of Laws Of 2025, Item 277) Cover The Following Types Of Activities: A) All Basic Physical Work On The Construction Site.4. Detailed Requirements Regarding The Implementation And Enforcement Of The Employment Requirement Based On An Employment Relationship Are Specified In The Contract Template Constituting Annex No. 2 To The Swz.4.1 During The Execution Of The Order, The Ordering Party Is Entitled To Carry Out Inspection Activities Towards The Contractor Regarding The Contractors Or Subcontractors Compliance With The Requirement Of Employing Persons Performing The Persons Specified In Point Under The Employment Contract. 3 Activities. The Ordering Party Is Entitled In Particular To: A) Request Documents Confirming Compliance With The Above-Mentioned Requirements And Assessing Them, E.G. Certified Copies Of Anonymized Employment Contracts (Devoid Of Employees Personal Data, Except For The Name And Surname, Date Of Conclusion Of The Employment Contract, Type Of Employment Contract, Working Hours), Declarations Of The Contractor Or Subcontractor On The Employment Under An Employment Contract Of Persons Performing Activities Containing Elements Of An Employment Relationship Or Certificates Of The Relevant Branch Of The Social Insurance Institution Confirming The Payment Of Social And Health Insurance Contributions In Respect Of Employment Under Employment Contracts, B) Requesting Explanations In The Event Of Doubts Regarding The Confirmation Of Compliance With The Above-Mentioned Requirements.
