The Subject Of The Order Is The Delivery Of A Portable (Mobile) Ultrasound Device For The Needs Of The Independent Public Municipal Health Care Center In Rzezawa In Order To Provide Primary Health Care By The Ordering Party.2. The Subject Of The Order Should Be Brand New (I.E. Unused, Not Regenerated, Not Used For Exhibition Purposes, Produced No Earlier Than In 2025), Of Good Quality, Complete, Free From Physical And Legal Defects, In Accordance With The Applicable Legal Provisions. It Should Have A Ce Declaration Of Conformity/Ce Certificate, Entry In The Register Of Medical Devices And Entities Responsible For Placing Them On The Market And Using Them In The Republic Of Poland, Guaranteeing The Safety Of Patients And Medical Staff.3. A Detailed Description Of The Subject Of The Order And Requirements Regarding The Minimum Technical And Operational Parameters Of The Subject Of The Order Are Included In Annex No. 2 To The Swz - Technical Parameters Form - Description Of The Subject Of The Order.4. The Subject Of The Order Includes: 1) Delivery, Installation And Commissioning Of The Ultrasound Device, 2) Provision Of Instruction In The Operation Of The Ultrasound Device On One Or Two Dates, Depending On The Needs Of The Ordering Party, After Prior Arrangement Of The Date Of Instruction And The Number Of People With The Ordering Party, 3) The Ordering Party Also Requires Full Implementation, Installation, Configuration And Integration Of The Provided License With The Ordering Partys It Systems, As Well As The Purchase And Delivery Of Appropriate Licenses If They Are Needed For The Correct Transmission Of Data From The Indicated Device. The Ordering Party Has The Following Specialized Systems: - His Amms By Asseco Sp. Z O. O. The Software Referred To Is Consistent With The Requirements Contained In The Regulation Of The Minister Of Health Of April 6, 2020 On The Types, Scope And Templates Of Medical Documentation And The Method Of Its Processing (Consolidated Text: Journal Of Laws Of 2024, Item 798, As Amended).5. All Requirements Specified In The Swz Are Minimum Requirements And Their Fulfillment Is Obligatory. Failure To Meet The Above-Mentioned Minimum Requirements Will Result In Rejection Of The Offer As Inconsistent With The Terms Of The Contract Pursuant To Art. 226 Section 1 Point 5 Of The Pzp.6 Act. The Contractor Is Obliged To Complete The Order On The Terms And Conditions Described In The Draft Provisions Of The Contract, Constituting Annex No. 6 To The Swz.
