Tenders Are Invited For Specialized Care Services For People With Mental Disorders

1. The Subject Of The Order Is A Service Consisting In Providing Minors Diagnosed With Mental Disorders Living In The Kowary Municipality With Specialized Care Services Tailored To Their Specific Needs Related To The Type Of Disease In The Form Recommended By A Doctor. These Services, In Accordance With Art. 3 Point 1 Letter A, B, C Of The Act Of August 19, 1994 On The Protection Of Mental Health (Journal Of Laws Of 2024, Item 917), Are Addressed To People: A) With Mental Disorders (Including People Showing Psychotic Disorders), B) With Intellectual Disability (Mental Retardation), C) Indicating Other Disturbances Of Mental Functions Which, Based On Current Medical Knowledge, Are Classified As Mental Disorders And Who Require Health Services Or Other Forms Of Support And Care Necessary For Life In A Family Or Community Environment.2. Specialized Care Services For People With Mental Disorders, Including People With Autism And People With Intellectual Disabilities - These Are Services Provided In The Place Of Residence And Adapted To The Specific Needs Resulting From The Type Of Their Disease Or Disability, Provided By People With Specialized Professional Training, In Accordance With The Regulation Of The Minister Of Social Policy Of September 22, 2005 On Specialized Care Services (Journal Of Laws Of 2005, No. 189, Item 1598, As Amended).3. Scope Of The Service Provided: A) Reading The Childs Documentation, Determining Directions And Schedule Of Therapeutic Activities, B) Establishing Cooperation With The Facility Where The Child Is Subject To Other Therapeutic Activities In Order To Ensure Coherence Of Interactions, C) Developing And Implementing An Individual Therapeutic Program With The Child, D) Assessing Progress And Analyzing The Effectiveness Of The Help Provided To The Child, E) Introducing Changes To The Program According To Current Needs Child, F) Providing The Childs Parents With Specialized Information Regarding The Activities Carried Out, Indicating Other Forms Of Comprehensive, Specialized Assistance Appropriate For The Child, 4. Service Hour = 45 Minutes, Includes Only The Actual Time Of Service Provision - It Does Not Include The Therapists Arrival Or Arrival To The Patient.5. Services May Be Provided On Any Day Of The Week, Including Public Holidays, At Hours Mutually Agreed By The Specialist And The Childs Parent.6. Services Should Be Provided In The Childs Place Of Residence (Kowary). With The Consent Of The Childs Parent And The Ordering Party, Services May Be Provided Outside The Childs Place Of Residence, If This Results From The Childs Identified Needs.7. The Contractor Is Obliged To Fulfill The Obligations Imposed By The Act Of May 13, 2016 On Counteracting Threats Of Sexual Crimes And Protection Of Minors - Before Starting The Service, The Ordering Party Requires The Contractor To Submit Written Information About Not Being Included In The Register Of Sexual Offenders (In The Form Of An Extract From The Register Of Sexual Of
