Tenders Are Invited For Delivery Of A Cryosurgery Device

1. The Subject Of The Order Is The Delivery, Assembly And Commissioning Of A Brand New Cryosurgery Device With The Necessary Equipment. As Part Of The Execution Of The Order, Training Instruction For The Ordering Partys Staff Should Be Provided In The Field Of Commissioning, Operation, Operation And Maintenance Of The Supplied Medical Equipment At The Place Of Installation Of The Device, Within The Time Period Specified For The Execution Of The Order. The Order Also Includes The Provision Of Warranty Service. 2. A Detailed Description Of The Subject Of The Order, Minimum Requirements, Functionalities And Parameters That The Cryosurgery Device Must Meet Is Included In The Description Of The Subject Of The Order, Which Constitutes Annex No. 1 To The Swz. This Description Should Be Read Together With Any Changes To The Content Of The Swz, E.G. As A Result Of Responses To The Contractors Inquiries.3. The Terms And Conditions Of The Contract Are Specified In The Draft Contractual Provisions Constituting Annex No. 6 To Swz4. The Subject Of The Order Must Be Brand New. It Cannot Be An Exhibition, Demonstration Or Reconditioned Product. It Must Be Free From Any Defects - Both Manufacturing And Legal. It Should Be Functional And Complete, I.E. Ready For Use After Installation In Accordance With Its Intended Purpose, Without The Need To Incur Additional Investments Or Purchase Elements And Accessories On The Part Of The Ordering Party.5. The Delivered Medical Equipment Must Have All Legally Required Current Declarations Of Conformity, Certificates, Markings And Documents Confirming Approval For Use In The Territory Of The Republic Of Poland, In Accordance With The Provisions Of The Act Of April 7, 2022 On Medical Devices (Consolidated Text: Journal Of Laws 2024, Item 1620, As Amended), And Also Meet Other Requirements, Including: Standards And Parameters Specified By The Ordering Party In Annex No. 1 To The Swz. The Cryosurgery Device Must Be Approved For Marketing And Use In The Republic Of Poland, In Accordance With Applicable Law. 6. The Subject Of The Order Must Comply With Applicable Regulations And Meet The Requirements Of Polish Standards Transposing European Harmonized Standards Or - In Their Absence - European Technical Approvals, Common Technical Specifications, Polish Standards Transposing European Standards, Polish Standards Implementing International Standards, Polish Standards, Polish Technical Approvals.7. The Subject Of The Order Must Be Accompanied By An Operating Manual In Polish For All Offered Components Of The Subject Of The Order. The Operating Manual Must Be Provided To The Ordering Party On The Day Of Receipt Of The Equipment In Paper And Electronic Versions In Pdf Format. 8. The Contractor Will Provide The Equipment With A Technical Passport, Which Will Contain Data And Parameters Of The Device: Type Of Device, Type, Serial Number, Manufacturers Name And Production Date, Etc. This Document Should Have A Place For Making Up-To-Date
