Construction Work – Expansion, Reconstruction And Modernization Of Hospital Building No. 5A For The Needs Of The Psychiatric Ward For Children And Adolescents. The Subject Of The Procedure Is The Award Of An Order For The Implementation Of An Investment Task Entitled: Expansion, Reconstruction And Modernization Of Hospital Building No. 5A For The Needs Of The Psychiatric Ward For Children And Adolescents, Which Will Be Implemented In The Design And Build System. 2. As Part Of The Subject Of The Order, The Contractor Will: 1) Develop Complete, Full-Discipline Design Documentation, Transfer The Proprietary Rights To The Design Documentation To The Ordering Party And Perform Authors Supervision On The Terms Described In The Contract Template - Annex No. 4 To The Swz; 2) Obtain, On Behalf Of And On Behalf Of The Ordering Party, All Necessary Arrangements, Opinions, Permits And Decisions Required By Law, Including In Particular The Decision Approving The Construction Design And Granting A Building Permit With A Clause Finality And Subject To Execution, In Order For The Contractor To Commence And Carry Out Construction Works; 3) Prepare Complete Documentation For The Interior Design And Equipment Of Hospital Building No. 5A, Transfer The Proprietary Rights To The Documentation Of Interior Design And Equipment To The Ordering Party And Ensure Authors Supervision; 4) Perform Construction Works Based On The Design Documentation And Obtain, On Behalf Of And For The Benefit Of The Ordering Party, A Final And Unconditional Decision On The Permit To Use Hospital Building No. 5A. 3. Contractor, Subject Of The Order Referred To In Section 2 Above, Will Be Implemented In Accordance With The Requirements Of The Ordering Party Specified In Detail In The Contract Template - Annex No. 4 To The Swz. 4. The Contractor Will Complete The Subject Of The Order In Accordance With The Requirements Of The Ordering Party Specified In This Documentation Of The Public Procurement Procedure, Including In Accordance With The Functional And Utility Program Entitled: Expansion, Reconstruction And Modernization Of Hospital Building No. 5A In Order To Adapt The Facility To The Needs Of The Stationary Department For The Treatment Of Neurotic Disorders For Children And Adolescents, Prepared By Kkad Sp. Z O. O. Based In Kraków; Opinion Of The Municipal Conservator Of Monuments Of June 5, 2018 - Letter No. Kz03.4120.6.174.2018.Mt And Decision No. Au2/6733/257/2018 Of July 18, 2018 On Determining The Location Of A Public Purpose Investment. 5. The Contractor Will Perform The Subject Of The Order In Accordance With Applicable Legal Regulations, As Well As In Accordance With The Best Knowledge And Experience, With The Utmost Care And In Accordance With The Principles Of Technical Knowledge And The Regulations And Standards In Force On The Date Of Final Acceptance, Which Also Means That If There Are No Detailed Descriptions Of The Construction Works Execution Technology Or No Other Activities Necessary For The Proper Performance Of The Construction Works Are Indicated, And A Given Technology Or Construction Art Suggests Their Execution, The Contractor Should Perform Such Works.
