The Subject Of The Order Is The Implementation Of An Investment In The Design And Build System Based On The Functional And Utility Program (Ffu), Including The Preparation Of Design Documentation And Construction Works Related To The Construction Of The Building Of The Community Self-Help Home In The Town Of Zagorzyn As Part Of The Project Entitled: Establishment Of The Community Self-Help Home In The Łącko Commune Co-Financed By The European Regional Development Fund Under The Program European Funds For Małopolska 2021-2027, Priority 0.5 European Funds Supporting Social Infrastructure, Measure 05.07 Infrastructure Related To Providing Care In The Local Community - Zit, Type A. Infrastructure Related To Providing Care To People Requiring Support Due To Age Or Disability Or Chronic Diseases. The Order Covers The Implementation Of A Comprehensive Project And Obtaining The Necessary Decisions, Opinions, Arrangements And Permits, Including Decisions On Building Permit Or Notification Of The Intention To Perform Construction Works And Execution Of Construction Works For The Comprehensive Construction Of The Building Of The Community Self-Help Home In Zagorzyn For Plot No. 383/3, 383/10 And 383/12, Together With The Necessary Infrastructure, Adaptation Of The Facility To Applicable Regulations, Adaptation Of The Building To The Needs Of People With Disabilities, Etc., Taking Into Account The Planned Functions Of The Facility For Daily Stays For Approx. 35 People, As Required Contained In The Regulation Of The Minister Of Labor And Social Policy Of December 9, 2010 On Community Self-Help Homes (Journal Of Laws Of 2020, Item 249) And The Development Of The Adjacent Area. The Building Of The Community Self-Help House Is A Two-Story Brick Building (Two Above-Ground Floors), Which Includes: - Administrative Part, - Therapeutic And Rehabilitation Part, - Social Part, - Kitchen/Dining Part, - Sanitary And Utility Facilities, - Landscaping (Parking, Sidewalks, Green Areas, Small Architecture) And Necessary Infrastructure. The Building Will Be Equipped With All The Required And Necessary For Proper Functioning. Installations And Equipment Necessary For The Operation Of The Facility. The Scope Of The Order Includes: - Preparation Of Design And Cost Estimate Documentation For The Construction Of The Community Self-Help Center Building (Including Construction And Executive Design, Detailed Technical Specifications For The Execution And Acceptance Of Construction Works, Bill Of Quantities, Investors Cost Estimate, Etc.); - Obtaining All Legally Required Administrative Decisions, Opinions, Arrangements And Permits Necessary To Design The Subject Of The Order, Including The Expertise Related To Obtaining A Derogation Due To The Lack Of Sufficient Water For Fire-Fighting Purposes And The Conditions Of Connection To Existing Networks And Stormwater Drainage. The Contractor Will Carry Out The Procedure For Obtaining A Final Decision On A Building Permit Or Reporting
