Health And Social Work Services - Purchase Of The Operation Of The Care Center Svalegangen, Including Nursing Care And Social Counseling For The Homeless - Re-Tender. The Tender Relates To The Purchasers Purchase Of The Operation Of The Care Center Swalegangen, Including The Implementation Of Social Work And Nursing Efforts Towards The Target Group. The Requirements Specification, Cf. Contract Annex 2, Contains A Detailed Description Of The Services Offered. The Purpose Of The Treatment Center Is To Establish Care Places For Citizens Experiencing Homelessness After Hospitalization. The Target Group Is Homeless Citizens Who Need Simultaneous Healthcare And Social Support And Care For A Shorter Period Immediately After Admission To One Of The Hospitals In Region Southern Denmark. In This Way, The Care Center Contributes To Prevention And Health Promotion In Relation To The Individual Patient. The Purpose Of The Stay Is For The Homeless Person To Experience An Improved Physical State Of Health In Relation To The Specific Illness And An Immediate Improved State Of Mental Health, As Well As For The Person To Have Better Contact With Existing Relevant Offers. The Offer For The Homeless Consists Of A Stay Of Up To 14 Days, Where The Homeless Are Offered Care, Counseling And Psychosocial Aftercare. Since The Care Center Is Organized As A Regional Treatment Center, Region Southern Denmark Has The Authority For The Operation Of The Care Home, And The Care Center Is Subject To The Agency For Patient Safety. The Care Center Is Not Covered By The Services Act, But It Is, On The Other Hand, Covered By The Health Act, Which Is Why The Care Center Is Not Subject To The Social Inspection. The Supplier Must Have Space At The Care Center For A Minimum Of Seven Residents At The Same Time, Of Which A Minimum Of Five Spaces Must Be Available For Region Southern Denmark. These Five Places Must Thus Be Reserved For Region Southern Denmark. The Supplier Is Responsible For Finding Suitable Premises Within Vejle, Kolding, Fredericia Or Middelfart Municipalities, Where The Care Center Is To Be Operated From The Start Of The Operating Period. The Supplier Must Operate The Care Center From And Including 1 July 2026 (I.E. Start Of Operation). If The Supplier Wishes To Use The Premises Leased By The Region (Sverigesvej 40 In Middelfart), The Start Of Operations Will However Be Postponed Until 8 July 2026. The Price Specified By The Tenderer In Contract Annex 3 Must Not Exceed Dkk 2.845 Million. Dkk Per Year. If This Happens, The Offer Will Be Considered Unconditional And Will Be Rejected. If The Price Is Adjusted In Subsequent Years In Accordance With The Contract, It Is Permitted That The Then Applicable Price May Exceed This Amount.
