Part 13.1. Task No. 1 - Sanitary Transport By A Specialized Team: 1) Consisting Of At Least Three People Authorized To Perform Emergency Medical Services, Including: A System Doctor And A System Nurse Or A Paramedic, 2) Required Availability: 7 Days A Week, 24 Hours A Day, 3) The Service Will Be Counted From The Place Where The S Team Reports At One Of The Hospitals - Regarding Both Locations Of The Ordering Party - Or At Another Point Designated By The Ordering Party To The Place Of Completion Of The Service. The Sanitary Transport Service Does Not Include Travel To The Premises Of The Ordering Party Or The Place Indicated By The Ordering Party As The Point (Place) Of Starting The Service And Does Not Include Returning To The Contractors Premises Or The Place Of Waiting For Vehicles After The Service Ordered By The Ordering Party Has Been Performed,4) The Arrival Of The S Team At The Place Of Call Should Take Place Immediately From The Moment Of Call, Within No Longer Than 35 Minutes To The Location In Kup And Within No More Than 45 Minutes To The Location In The Room,5) Transport Service Will Consist In Picking Up The Patient From The Ward, Taking Him To The Vehicle, Transporting Him To The Address Indicated In The Transport Order, Taking The Patient: For Examination, Consultation, (In The Event Of Transfer To Another Hospital - Taking The Patient To The Emergency Room),6) Team S Can Only Provide Transport Services In Conditions Of Immediate Life Threat. In The Event Of A Direct Threat To Life, The Contractor Will Be Obliged To Carry Out The Procedure Of Unblocking The Airway By Endotracheal Intubation, If Necessary.7) The Order Should Include: Transport Priority: Urgent/Planned, Name And Surname Of The Doctor With Whom The Transfer/Consultation Of The Patient Was Arranged, Name, Surname And Pesel Number Of The Patient; Place Of Start Of Transport;  Place Of Transport Destination;  Determination Of The Type Of Transport;  Date, Hour And Minute Of The Start Of The Order;  Reason For Transport;  Patients Position During Transport;  Signature And Stamp Of An Authorized Employee Of The Ordering Party. 8) Acceptance Of An Order For A Transport Service Will Be Based On A Prior Telephone Notification By Authorized Staff Of The Ordering Party, To The Telephone Number Indicated By The Contractor.
