The Subject Of The Order Is The Provision Of Care Services For 40 People, Residents Of The Świdnik Municipality, In Need Of Support In Everyday Functioning, At Risk Of Being Placed In 24-Hour Institutions, People With Disabilities, Adults And Children, People At Risk Or Affected By Poverty Or Social Exclusion - Participants Of The Project Development Of Social Services In Lom In The Głusk And Świdnik Communes Implemented Under The European Funds For The Lublin Voivodeship Program 2021-2027 Co-Financed By The European Social Fund Plus. The Project Is Implemented In Partnership With The Lublin Metropolitan Area Association And The Głusk Commune On The Basis Of A Partnership Agreement Of May 28, 2025. Support Will Be Provided First To People Up To 65 Years Of Age Who Need Support In Everyday Functioning And People With Disabilities. Care Services Include Assistance In Meeting Everyday Life Needs, Hygienic Care, Care Recommended By A Doctor And, If Possible, Ensuring Contacts With The Environment - Art. 50 Section 3 Of The Act Of 12 March 2004 On Social Assistance. The Scope Of Care Services Tailored To The Individual Needs Of The Project Participant Provided At The Place Of Residence Includes In Particular: 1) Meeting The Daily Life Needs Of The Patient, I.E.: A) Providing Meals For The Patient: Cooking Or Bringing Dinner, Feeding, B) Assistance In Keeping The Person Under Care Clean: The Patients Toilet, Including Hygiene Of The Oral Cavity, Fingernails And Toenails, Making The Bed, Laying The Patient Down And Preventing The Formation Of Bedsores, Assistance With Physiological Activities, C) Maintaining Cleanliness In The Rooms Used By The Person Benefiting From Assistance And Sanitary Equipment And Sanitary Facilities D) Washing Light Clothing And Personal Underwear, E) Shopping For Basic Food And Household Products In The Nearest Store, F) Collecting And Delivering Aid In Kind, Food Parcels G) Walking Together And Contact With The Surroundings, 2) Visits With People Under Care In: Health Care Facilities, Institutions, Offices, Etc., A) Cooperation With A Health Clinic And A Community Nurse, Ordering Medical Visits, B) Collecting And Dispensing Prescriptions (For People Not Leaving Home), C) Handling Official Matters3.3. Estimated Number Of Hours Of Care Services Provided: 28,013 Hours. 1) Frequency And Length Of Support: On Average 700 Hours For 40 People Over 26 Months, I.E. On Average 26-27 Hours/1 Month/1 Person.2) Number Of People Under Care - 40 People. Each Person Will Receive An Assigned Caregiver Who Will Help Them Meet Their Daily Needs. An Hour Of Service Provision Means A Clock Hour (60 Minutes) Of Providing The Service At The Clients Place Of Residence, As Well As Performing Commissioned Activities For Him Outside The Place Of Residence, Such As Handling Official Matters Or Making Necessary Purchases, Etc.
