The Subject Of The Tender Is The Selection Of A Contractor Who Will Provide The Ordering Party With Services Consisting In The Collection (Including Loading), Transport And Disposal Of Medical Waste From Group 18 01, I.E. Mainly 18 01 02* And 18 01 03* In Accordance With The Regulation Of The Minister Of Climate Of January 2, 2020 On The Waste Catalog (Journal Of Laws Of 2020, Item 10) From The District Hospital. Dr. Tytus Chałubiński In Zakopane Together With The Specialist Clinic Located At Kamieniec 10, 34-500 Zakopane, Maintaining Continuity Of Collection So That The Waste Does Not Pose A Sanitary And Epidemiological Threat To The District Hospital In Zakopane With Its Registered Office At Ul. Kamieniec 10 For A Period Of 12 Months, In Accordance With Applicable Law.2. The Ordering Party Informs That The Quantity Of Medical Waste That Is The Subject Of This Tender Procedure Given Below Is An Estimated Amount, Which May Change During The Duration Of The Contract, Reduced By No More Than 20% Depending On The Actual, Current Needs Of The Ordering Party, Which Is Also The Maximum Order Quantity, And Thus: 1) Waste Per Month In An Average Quantity - 6,300 Kg, Which Gives An Estimated Quantity Of No More Than 75,600 Throughout The Duration Of The Contract. Kg. The Ordering Party Reserves The Right To Withdraw From The Contract If The Order Is Completed In Terms Of Quantity (Specified Above) Earlier Than The Period For Which The Contract Was Concluded. Moreover, The Ordering Party Stipulates That The Contract Will Expire After The Period For Which It Was Concluded, Regardless Of The Number Of Services Provided, And The Ordering Party Will Not Be Charged With The Costs Of Unrealized Services.3. The Ordering Party Reserves, And All Contractors Of The Contract (Contractor, Subcontractors, Consortium Members And Others) Undertake To Collect (Including Loading), Transport And Neutralize Medical Waste That Is The Subject Of This Tender Procedure Successively According To The Schedule Specified In Point 4 Below In Accordance With The Provisions Of The Currently Applicable Legal Provisions In This Regard, In Particular The Act Of December 14, 2012 On Waste (Consolidated Text Journal Of Laws Of 2023, Item 1587, As Amended) And Implementing Acts Issued To The Act.4. Collection, Loading, Transport And Disposal Of Medical Waste From The Places Specified Above In Point 1 Will Take Place Three Times A Week, I.E. On Monday, Wednesday And Friday On Working Days From 8:00 A.M. To 2:00 P.M. (And Additionally On A Previously Agreed Date During Holidays And Non-Working Days). The Person Authorized To Transfer Waste Is An Employee Of The Hospital Hygiene Department.5. The Ordering Party Reserves The Right To, And All Contract Executors (Contractor, Subcontractors, Consortium Members And Others) Declare That They Have The Necessary Authorizations To Perform The Service That Is The Subject Of The Order, I.E. Collection, Transport And Disposal Of Medical Waste In Accord
