1) The Subject Of The Order Is The Provision Of Specialized Care Services For People Living In The Radzyń Podlaski Commune. The Contractor(S) Will Perform The Services At The Place Of Residence Of The Person Using The Services.2) The Order Is Divided Into 13 Parts. It Includes Specialized Care Services Provided To Children And Adolescents With Mental Disorders, Consisting Of Access To Rehabilitation, Revalidation And Educational Activities. The Provision Of Services That Are The Subject Of This Order Will Be Carried Out In Accordance With, In Particular, The Provisions Of The Act Of August 19, 1994 On Mental Health Protection (Journal Of Laws Of 2024, Item 917, As Amended), The Act Of March 12, 2004 On Social Assistance (Journal Of Laws Of 2024, Item 1283, As Amended). The Scope Of Support Was Calculated Based On The Initial Diagnosis And The Number Of Hours Of Support Granted To The Patients In 2025. 5) The Task Of The Contractor(S) Of This Order Is To Provide Specialized Care Services Based On The Issued Administrative Decision. The Contractor May Provide Support 7 Days A Week (After Prior Arrangement Of The Date For Carrying Out The Services With The Guardian Of The Person Who Requires Support). The Hours During Which Services Are To Be Provided Should Be Adapted To The Needs Of The Recipients. 6) The Given Number Of Hours Within A Given Part Of The Order Is The Estimated Number Of Hours Of Support That Can Be Granted And Provided During The Order Execution Period. The Ordering Party Allows For Increases And Decreases In The Number Of Hours For Individual Parts Of The Order. The Ordering Party Included The Conditions For Making Changes In The Contract Template. The Ordering Party Will Pay The Contractor Remuneration For The Hours Actually Performed.
