The Subject Of The Order Is The Service Of Preparing Chimney Inspection Opinions, Including Checking The Technical Condition Of Smoke, Exhaust And Ventilation Chimney Flues In Vacant Premises Located In Buildings Constituting The Municipal Resource Of The Commune Managed And Administered By The Housing Management Department, Towarzystwo Budownictwa Społecznego In Częstochowa Spółka Z O. O. Street Polish Organization Military 24, 42-200 Częstochowa. The Provision Of One Service Should Be Understood As Checking The Technical Condition Of Chimney, Smoke, Exhaust And Ventilation Ducts And Preparing Chimney Inspection Opinions (Including Possible Implementation Recommendations), Before And After The Renovation Of A Vacant Premises. The Services Will Be Performed On The Basis Of Orders Issued By The Ordering Party, Containing The Exact Address Of The Premises And Building. The Contractor Performs Activities Related To The Implementation Of The Subject Of The Order Using Its Own Employees, Materials, Tools And Means Of Transport. Estimated Number Of Premises To Be Checked – 130 Units2. A Detailed Description Of The Subject Of The Order Is Specified In The Technical Specification Constituting Annex No. 9 To The Swz And In The Contract Template Constituting Annex No. 6 To The Swz. In The Offer Price, The Contractor Must Include All Costs Necessary To Complete The Order, Resulting Directly From The Swz, As Well As Those Not Included In It, And Without Which The Order Cannot Be Performed.3. The Contractor Is Obliged To Comply With Occupational Health And Safety And Fire Protection Regulations As Well As To Provide Equipment And Clothing For Persons Participating In The Task.4. Acting In Accordance With Art. 95 Section 1 And 2 Point 1 Of The Public Procurement Law, The Ordering Party Requires The Contractor Or Subcontractors To Employ, Under An Employment Relationship, Persons Performing The Following Activities Within The Scope Of Contract Execution, If The Performance Of These Activities Involves Performing Work In The Manner Specified In Art. 22 § 1 Of The Act Of June 26, 1974 - Labor Code, Including: 1) The Ordering Party Requires That All Persons Implementing The Subject Of The Order, Who Will Perform Actual Activities Related To The Performance Of Works, Consisting In Preparing Chimney Inspection Opinions, Including Checking The Technical Condition Of Smoke, Exhaust And Ventilation Chimney Ducts, Be Employed On The Basis Of An Employment Relationship, 2) The Contractor Or Subcontractor Employs The Above-Mentioned Persons For The Period Of Execution Of The Order. If The Employment Relationship Is Terminated Before The End Of This Period, The Employee Undertakes To Immediately Employ Another Person In His Place. The Obligation Referred To In The Previous Sentence Will Not Arise In The Event Of Termination Of The Employment Relationship Before The End Of This Period And Applies To The Person Whose Role In The Execution Of The Order Has Been Completed,
