Waste And Rubbish Containers And Bins – Purchase And Installation Of Containers For The Selective Collection Of Municipal Waste As Part Of The Project Development Of The System For Selective Collection Of Municipal Waste Feld.02.13-Iz.00-0020/24-00 - Part Four. The Ordering Party Pursuant To Art. 139 Section 1 Of The Public Procurement Law, Reserves The Right To First Examine And Evaluate The Offers, And Then Subjectively Qualify The Contractor Whose Offer Was Rated The Highest In Terms Of The Lack Of Grounds For Exclusion And Compliance With The Conditions For Participation In The Proceedings. The Contractor Is Obliged To Submit With The Offer: 1) Offer Form, 2) Price Form, 3) Declaration In The Form Of The Single European Procurement Document (Also Called Sed) Form, Which Constitutes An Annex To The Swz. The Information Contained In The Jedz Form Constitutes Evidence Confirming The Lack Of Grounds For Exclusion From The Procedure And The Fulfillment Of The Conditions For Participation In The Procedure On The Date Of Submission Of Offers - Submitted In The Original Electronic Form, 4) The Contractors Declaration Regarding The Sanction Condition Pursuant To Art. 5K Of Council Regulation (Eu) No. 833/2014 Of 31 July 2014, As Amended, Concerning Restrictive Measures In Connection With Russias Actions Destabilizing The Situation In Ukraine - In Accordance With The Template Constituting The Annex To The Swz; 5) In The Case Of Contractors Jointly Applying For The Contract, The Declaration Specified In Point 3 And 4 Are Submitted By Each Of These Contractors (Also Applies To Partners Of A Civil Partnership). 6) In The Case Of An Entity Providing Resources, The Declarations Referred To In Point 3 And 4 Are Submitted By Each Of These Entities. 7) Declaration Of Contractors Jointly Applying For The Award Of The Contract Under Art. 117 Section 4 Of The Public Procurement Law. 8) Power Of Attorney - If Applicable, 9) Declarations And/Or Documents On The Basis Of Which The Ordering Party Will Assess The Effectiveness Of The Reservation Of Information Contained In The Offer, Constituting A Business Secret Within The Meaning Of The Provisions On Combating Unfair Competition (If The Contractor Reserves Such Information). 1. Documents Submitted Upon Request: Pursuant To Art. 126 Section 1 Of The Public Procurement Law, The Ordering Party Calls On The Contractor Whose Offer Was Rated The Highest To Submit, Within A Specified Period Of Time, Not Less Than 10 Days, Subjective Evidence Valid On The Date Of Submission. In Order To Confirm The Lack Of Grounds For Excluding The Contractor From Participation In The Procedure, The Ordering Party Requests The Following Documents: 1) Information From The National Criminal Register In The Scope Of: A) Art. 108 Section 1 Point 1 And 2 Of The Public Procurement Law, B) Art. 108 Section 1 Point 4 Of The Public Procurement Act, Regarding The Prohibition To Apply For A Public Procurement Contract As A Penal Measure, Prepared No Earlier Than 6 Months Before Its Submission; 2) Declaration Of The Contractor, Within The Scope Of Art. 108 Section 1 Point 5 Of The Public Procurement Law, On Not Belonging To The Same Capital Group Within The Meaning Of The Act Of February 16, 2007 On Competition And Consumer Protection (Journal Of Laws Of 2024, Item 1616), With Another Contractor Who Submitted A Separate Offer, A Partial Offer Or An Application To Participate In The Procedure, Or A Declaration Of Membership In The Same Capital Group Together With Documents Or Information Confirming The Preparation Of The Offer, Partial Offers
