The Subject Of The Order Is The Rental Of 5 Vehicles Along With Servicing, Insurance And Other Services Specified In The Detailed Description Of The Order (Hereinafter: Sopz). 2. A Detailed Description Of The Subject Of The Order Is Included In Annex No. 1 To The Swz - Sopz, Hereinafter Referred To As Sopz And In The Contract Template, The Template Of Which Is Annex No. 7 To The Swz (Hereinafter Referred To As: Contract Template Or Agreement). 3. The Proceedings Are Conducted In Polish. 4. The Offer Consists Of: 1) Offer Form, 2) Price Form. The Following Must Be Submitted Along With The Offer: 1) Edz Declaration; 2) Power Of Attorney Indicating The Representative Of The Contractors Acting Jointly (In The Case Of A Bid Submitted By A Consortium); 3) Power Of Attorney To Sign The Offer (If An Attorney Is Used); 4) The Declaration Referred To In Chapter Vi Section 9 Swz (Appendix No. 3 To The Swz - In The Case Of Contractors Acting Jointly) 5) Obligation Of The Entity Providing Resources Prepared On The Basis Of The Template Constituting Appendix No. 10 To The Swz. 6) Evidence, In Particular The Means Of Evidence Referred To In Art. 104-107 Of The Public Procurement Law, Confirming That The Solutions Proposed By The Contractor Equally Meet The Requirements Specified In The Sopz (In The Case Of Offering Equivalent Solutions); 7) Declaration Of The Entity Providing Resources Regarding The Grounds For Exclusion Under Art. 5K Of Regulation 833/2014 And Art. 7 Section 1 Of The Act Of April 13, 2022 - If The Contractor Relies On The Resources Of Another Entity - Based On The Template Constituting Annex No. 11 To The Swz. 5. The Obligation Of The Entity Providing Access (Chapter Vii, Section 2 Of The Swz), Powers Of Attorney Or Subjective Means Of Evidence Should Be Submitted In Accordance With The Provisions Of The Regulation Of The Prime Minister Of December 30, 2020 On The Method Of Preparing And Transmitting Information And Technical Requirements For Electronic Documents And Means Of Electronic Communication In Public Procurement Or Competition Proceedings. 6. Certification Of Compliance With The Original Is Made By Signing With A Qualified Electronic Signature. The Certification Is Made By A Notary Or The Contractor (Member Of The Consortium, Entity Providing Resources - Respectively In The Scope Of Documents Relating To Each Of Them), And In The Case Of A Power Of Attorney, The Certification Is Made By A Notary Or The Principal.
