4.1. The Subject Of The Order Is The Provision Of Collection, Transport And Disposal Services Of Medical Waste From Subgroup 18 01 - This Means Waste From Perinatal Care, Diagnosis, Treatment And Preventive Medicine Type 18 01 02*, 18 01 03*, 18 01 82, 18 01 09 (In Accordance With The Waste Catalog Specified In The Regulation Of The Minister Of Climate Of January 2 2020 Regarding The Waste Catalogue). The Total Estimated Monthly Weight Of Waste From Subgroup 18 01 (18 01 02*, 18 01 03*, 18 01 82, 18 01 09) Is Approximately 2,600 Kg. The Given Quantity May Be Decreased Or Increased Depending On The Current Needs Of The Ordering Party. The Total Quantity Expected Will Be 62,400 Kg.4.2. The Contractor, As The Transporter And Recipient Of Waste, Is Obliged To Have An Entry In The Bdo Register (Bdo Registration Number) And To Directly Participate In The Electronic Records Of This Waste, In Accordance With Bdo Requirements. 4.3. The Contractor Will Confirm The Receipt Of Waste Using The Bdo Electronic System (Waste Database) On The Waste Transfer Card (Kpo) Created In The System By The Ordering Party. The Entries Kept In The Waste Transfer Card Will Be The Basis For Issuing A Vat Invoice For The Service - Settlements For The Service Provided Will Be Made At The End Of Each Calendar Month.4.4. The Contractor Is Obliged To Execute The Order In Accordance With Generally Applicable Law, Including The Act Of December 14, 2012 On Waste.4.5. Pursuant To Art. 95 Section 2-3 Of The Act Of December 14, 2012 On Waste, The Ordering Party Requires That Infectious Medical Waste Be Neutralized By Thermal Transformation In Hazardous Waste Incineration Plants. At The Same Time, Their Disposal In Waste Co-Incineration Plants Is Prohibited.4.7. The Ordering Party Does Not Divide The Order Into Parts And Therefore Does Not Allow The Submission Of Partial Offers. Offers That Do Not Include The Full Scope Of The Subject Matter Of The Contract Will Be Rejected. Reasons For Not Dividing The Order Into Parts - It Is Not Possible To Divide The Order Into Parts, Because In Technical, Organizational And Economic Terms It Creates An Inseparable Whole.4.8. The Ordering Party Requires The Contractor And Subcontractor, Pursuant To Art. 95 Section 1 Of The Act, So That Persons Performing The Following Activities Within The Scope Of The Contract, I.E. Activities: Transport Of Waste (Including Loading And Unloading), Transport And Disposal Of Waste, Are Employed On The Basis Of An Employment Relationship Within The Meaning Of The Act Of June 26, 1974 - Labor Code (Consolidated Text: Journal Of Laws Of 2020, Item 1320, As Amended). The Method Of Verifying The Employment Of Persons Referred To In Art. 95 Section 1 Of The Act And The Ordering Partys Rights To Control The Contractors Compliance With The Requirements Related To The Employment Of These Persons And The Sanctions For Failure To Meet These Requirements Are Specified In The Provisions Of The Contract Template Constituting Annex N
