The Subject Of The Order Is The Purchase And Delivery Of Medical Equipment - An Ultrasound Device And An Eeg Device For The Neurology Department With The Stroke Subunit Of The Multi-Specialty District Hospital Sa In Tarnowskie Góry. The Order Was Divided Into 2 Parts: Part A – Ultrasound Device2. The Detailed Scope Of The Subject Of The Order And The Description Of The Ordering Partys Requirements In Terms Of Implementation And Acceptance Are Specified In: 1) The Offer Form With A Description Of The Subject Of The Order - Annex No. 1 To The Swz2) Draft Provisions Of The Contract - Annex No. 3 To The Swz.3. The Required Warranty And Warranty Period For The Completed Subject Of The Contract Is At Least 24 Months From The Date Of Signing The Fault-Free Delivery And Acceptance Protocol By The Ordering Party, The User (I.E. Wsp Sa) And The Contractor.4. The Ordering Party Requires The Contractor To Provide Evidence Specified In The Description Of The Subject Of The Order Constituting Annex No. 1 To The Tor, Confirming That The Offered Supplies Meet The Requirements Specified By The Ordering Party.5. The Contractor Submits The Evidence Together With The Offer.6. If The Contractor Has Not Submitted The Evidence In Question Or The Evidence Submitted Is Incomplete, The Contracting Authority Will Request Their Submission Or Completion Within The Specified Deadline.7. The Call Procedure Referred To In Section 6 Shall Not Apply If The Evidence Serves To Confirm Compliance With The Features Or Criteria Specified In The Description Of The Offer Evaluation Criteria Or, Despite The Submission Of The Evidence, The Offer Is Rejected Or There Are Grounds For Invalidation Of The Proceedings.8. The Ordering Party May Request Explanations From Contractors Regarding The Content Of The Evidence In Question.9. The Project (Investment) Is Co-Financed From A Targeted Subsidy From The State Budget Created To Support The Implementation Of Development Policy Activities, Constituting The Own Investment Task Of The Local Government Unit By Decision Of The Minister Of Finance, Investment And Development.
