The Subject Of The Order Is The Purchase Of The Service Of Employing A Medical Examiner To Determine Fitness For Military Service In 2026 For The Needs Of The Wcr In Puławy And The Purchase Of The Service Of Contracting A Physician To Provide Health Services In The Field Of Preventive Health Care For The Needs Of The 41St Air Training Base In Dęblin In 2026. The Subject Of The Order Is Divided Into Two Independent Parts: ➢ Task I - Purchase Of The Service Of Employing A Medical Examiner To Make Judgments. Ability To Serve In The Military In 2026 For The Needs Of The Wcr In Puławy, The Ordering Party Allows The Submission Of Partial Offers, Which Should Be Understood That The Contractor May Submit An Offer For The Full Scope Of One Or Two Tasks (Parts) Of The Order. Each Complete Task Is Part Of The Order. The Ordering Party Allows The Possibility Of Submitting An Offer For All Or Selected Task (Part) Of The Order. Each Task (Part) Of The Order Will Be Assessed Separately. Offers That Do Not Include The Full Scope Of The Subject Matter Of The Contract Specified In Individual Tasks Will Be Rejected Pursuant To Art. 226 Section 1 Point 5) Of The Public Procurement Law 2. A Detailed Description Of The Subject Of The Contract, Task I, • Annex No. 5A To The Swz, I.E. Designed Provisions Of The Contract - For Task I,3. The Scope Of Services Includes: 3.1. Task I - Medical Examiner To Assess Fitness For Military Service In 2026 For The Needs Of The Wcr In Puławy: Basic Tasks Performed In The Position: 1. Conducting A Medical Interview.2. Conducting A Physical Examination.3. Analysis Of Medical Documentation Provided By The Examined Person.4. Issuing A Decision On Fitness For Military Service.5. Keeping Records Of Examined Persons.6. Maintaining A Book Of Medical Certificates In Paper Or Electronic Form.7. Other Services In The Field Of Determining Fitness For Military Service. In The Tender Form, The Contractor Will Declare His Experience In Adjudicating In Years (Min. 1 Year). Additional Arrangements: A) The Service Is Provided 2 Days A Week Plus One Saturday A Month (If Necessary), I.E. A Total Of 98 Days In 2026 At The Headquarters Of Wcr Puławy, Ul. Marszałka Józefa Piłsudskiego 93, 24-100 Puławy. B) Payment For The Service Provided Will Be Made By Transfer To The Bank Account Within 30 Days (Thirty Days) Of The Ordering Partys Receipt Of The Schedule Of Services Provided And A Correctly Issued Invoice. Payment Will Be Made To The Contractors Bank Account. C) The Selected Contractor Should Have The Right To Practice Medicine In The Territory Of The Republic Of Poland Issued By The District Medical Chamber
