The Subject Of The Order Is The Provision Of Psychological Support Services: 1. For Home Hospice Residents (People Aged 65 And Older) - 24 People Per Month For 3 Months Each; 2. For Informal Caregivers Of Hospice Patients (Helping Caregivers Cope With Emotions, Stress And Challenges Related To The Care Of Dependent People). Care Provided In The Patients Place Of Residence. Psychological Services: 1) Will Be Provided 24/7 Access To Services Provided By A Psychologist. 2) For Each Person In Need Of Support, 2 Consultations Per Month Will Be Provided. 3) For Informal Caregivers Of Hospice Patients, 1 Consultation Per Month Will Be Provided. 4) Services Will Be Supported By The Use Of Remote Channels For Contact With The Patient - Telemedicine. 5) Cooperation Within The Therapeutic Team In Terms Of Qualifying The Patient And Creating An Individual Patient Care Plan (Together With A Doctor, Nurse, Rehabilitator, And Project Administrator). 6) The Bidder Provides His Own Transport.
