Tenders Are Invited For Doctors Home Visits To The Patients Place Of Residence As Part Of Home Hospice Care - People Aged 65 And Older (24 People Per Month For A Period Of 3 Months Each)

The Services Provided By The Doctor Will Include Activities In The Field Of: 1) Ensuring A Dignified Life In Illness And Dying With Dignity, Pain Treatment In Accordance With Current Medical Knowledge Using Drugs Available In The Territory Of The Republic Of Poland, 2) Treatment Of Other Somatic Symptoms, Prevention Of Complications, Tests Ordered By A Doctor Employed In The Hospice, And Medication Prescription. 3) 24/7 Access To Services Provided By A Doctor Will Be Provided. Two Medical Consultations Per Month Will Be Provided For One Person In Need Of Support4) The Services Will Be Supported By The Use Of Remote Channels For Contact With The Patient - Telemedicine.5) Cooperation Within The Therapeutic Team In Terms Of Qualifying The Patient And Creating An Individual Patient Care Plan (Together With A Nurse, A Rehabilitator, A Psychologist And The Project Administrator).6) The Offeror Provides Its Own Transport To The Patient.
