Nursing Services: 1) Specialized Care For The Elderly, Chronically Ill Requiring Specialized Long-Term Care Within The Framework Of Home Hospice For 24 People Per Month For A Period Of 3 Months Each. 2) 24/7 Access To Services Provided By A Nurse Will Be Provided. 3) 4 Visits Per Week Will Be Made For One Person In Need Of Support, Depending On The Current Number Of Patients Covered By Home Hospice Services. 4) Services Will Be Supported By The Use Of Remote Channels For Contact With The Patient - Telemedicine.5) Cooperation Within The Therapeutic Team In Terms Of Qualifying The Patient And Creating An Individual Patient Care Plan (Together With A Doctor, Rehabilitator, Psychologist And Project Administrator).6) The Bidder Provides His Own Transport To The Patient.
