Hospital Facilities Construction Work - Modernization Of Rooms On The Lower Ground Floor Of Building K Into Diagnostic Laboratories For Magnetic Resonance And Angiograph With Insulation Of A Part Of The Elevation Of The St. Adathers Hospital Building. 1. The Subject Of The Order Includes, Among Others: Carrying Out Design Works And Construction And Assembly Works, Including The Delivery Of An Angiograph And The Preparation Of Rooms For The Installation Of Magnetic Resonance Imaging, Consisting In Adapting The Rooms Of Building K In The Hospital. Saint Wojciech In Gdańsk To Improve The Availability And Quality Of Cardiac Care. As Part Of This Investment, It Is Planned To Reconstruct And Modernize Some Of The Rooms And Hospital Infrastructure Located On Level 0 Of Buildings K And A. Design And Build Procedure. 2. The Subject Of The Order Consists Of One Part. The Detailed Scope And Method Of Implementing The Subject Of The Order Are Set Out In Annex No. 2 To The Swz - Description Of The Subject Of The Order (Called Opz) Together With The Annexes. The Terms And Conditions Of The Order Are Included In Annex No. 4 To The Swz - Draft Contract As Attachments. 3. The Contractor Will Provide A Warranty For Construction And Installation Works Covering The Subject Of This Order For A Minimum Period Of 36 Months And A Maximum Of 60 Months (From The Date Of Final Acceptance Of The Works). The Definitions Of Acceptance And Warranty Are Described In Annex No. 4 To The Swz. The Warranty Period Is One Of The Criteria For Evaluating Offers. 4. The Contractor Is Obliged To Conclude, For The Duration Of The Contract Regarding The Order In Question, No Later Than The Day Preceding The Day On Which The Handover Of The Construction Site Is To Take Place, An Insurance Contract Or Contracts For Insurance Against All Risks And Liabilities Related To The Implementation Of The Contract, And To Pay The Due Insurance Premiums On Time And Within The Payment Deadline Indicated On The Policy, To The Extent Described In Point No. 16 Of Annex No. 4 To The Swz. 5. Delivery Of The Angiograph:  A Detailed Description Of The Purchase Of The Angiograph Is Provided In Annex 2 To The Agreement - Detailed Specification Of Medical Equipment, Which The Contractor Completes And Attaches To The Offer. By Completing The Indicated Items, The Contractor Confirms That It Meets The Requirements Specified By The Ordering Party,  The Contractor Will Provide A Warranty For The Angiograph For A Period Of 36 Months,  It Is Agreed That The Offered Items Of The Order Must Be Brand New, Complete, Of A High Standard Both In Terms Of Quality And Functionality, And Free From Material, Construction And Legal Defects,  The Subject Of The Order Must Be Consistent With The Act Of April 7, 2022 On Medical Devices (I.E. Journal Of Laws 2022.974 Of 2022/05/09). 6. Equivalent Solutions: In All Provisions Of The Swz And Its Annexes, Which Indicate Trademarks, Parameters, Sources Or A Specific Process That Characterizes The Products Supplied By A Specific Contractor (Names Of Manufacturers Or Devices), In Accordance With Art. 99 Section 5 Pzp And When The Subject Of The Contract Is Described By Reference To Standards, Technical Assessments, Technical Specifications, Reference Systems, In Accordance With Art. 101 Section 4 Of The Public Procurement Law, The Ordering Party Allows Equivalent Solutions. 7. The Ordering Party Does Not Reserve The Right To Apply For The Order Only By Contractors Referred To In Art.
