Tenders Are Invited For Service Providing 24-Hour Veterinary Care For Service Dogs And Dogs Withdrawn From Service Remaining In The Inventory Of Kmp Katowice
The Subject Of The Order Is A Service Providing 24-Hour Veterinary Care For Service Dogs And Dogs Withdrawn From Service Remaining At The Kmp Katowice. Distance Between The Unit Katowice Ul. Kochłowicka 5 And The Veterinary Clinic (Place Of Service Provided By The Contractor) Cannot Be Longer Than 50 Kilometers Along The Shortest Route According To Google Maps. The Veterinary Clinic Is Obliged To Provide The Services Indicated In The Price Form During Working Hours. The Ordering Party Requires That The Clinic Be Open At Least 8 Hours A Day From Monday To Friday. In Cases Of Sudden Illness, The Veterinary Clinic Should Provide Services To The Ordering Party 24 Hours A Day. The Contractor Undertakes To Be On Call And Start Activities Related To The Treatment Of The Animal Within 2 Hours Of Receiving Information About An Emergency Event Requiring Veterinary Intervention At The Dogs Place Of Stay. Medicines Administered To Service Animals During A Visit (As Well As Those Prescribed For Further Administration Outside The Clinic) Are Understood As A Broadly Understood Veterinary Service. The Veterinary Service Provided To A Dog Withdrawn From Service Is Confirmed On The Treatment Card By The Head Of The Prevention Department In The Unit. The Protocol Confirming The Visit Will Be A Printout From The Clinic, E.G. A Visit Card Prepared By A Given Clinic And Confirmed By The Signature Of The Veterinarian, The Service Dog Guardian/Coordinator And The Head Of The Prevention Department Of The Unit. The Gross Offer Price Calculated On The Basis Of The Price Form In Accordance With Section Xvii Of The Swz, Entered Into The Offer Form Constituting Annex No. 1 To The Swz, Will Be Used By The Ordering Party To Select The Most Advantageous Offer. The Settlement Of Contract Stages Will Be Based On The Services Actually Provided In Accordance With The Offered Unit Price For A Given Type Of Service Given In The Price Form In Accordance With Annex No. 1A To The Swz. The Value Of The Contract Will Be The Amount That The Ordering Party Intends To Allocate For The Implementation Of The Order, I.E. Pln 90,000.00. For Services Other Than Those Indicated In The Price Form, Prices Will Be Applied According To The Contractors Current Price List Of Veterinary Services. The Contractor Will Be Obliged To Provide A Price List At The Request Of The Ordering Party. Detailed Information Regarding The Description Of The Subject Of The Order And Its Scope Can Be Found In: A) Annex No. 1 To The Swz - Offer Form, B) Annex No. 1A To The Swz - Price Form, C) Annex No. 2 To The Swz - Draft Provisions Of The Contract. The Ordering Party Allows Subcontractors. If The Contractor Uses A Subcontractor, The Ordering Party Requests An Indication Of The Part Of The Order That It Intends To Entrust To The Subcontractor And, If Already Known, The Names Of The Subcontractors Companies Along With Contact Details (Appendix No. 1 To The Swz - Offer Form).
