1. Subject And Scope Of The Order For Part I - Prevention Of Animal Homelessness In The Żyrardów Commune.2. The Contractor Will Provide Services In A Medical Facility That Meets The Requirements Of The Act Of December 18, 2003 On Animal Medical Facilities (Journal Of Laws 2019.24). Before Signing The Contract, The Contractor Will Submit An Appropriate Declaration That The Animal Clinic Provides Veterinary Services Covered By This Order And Meets The Requirements Described In The Law. The Ordering Party Requires That The Services Performed By The Contractor In Part I Be Performed In A Medical Facility In The City Of Żyrardów, Due To The Availability And Nature Of The Subject Of The Order.3. The Contractor Will Be Obliged To Keep Medical And Veterinary Documentation Regarding The Treatments Performed, In Accordance With The Regulation Of The Minister Of Agriculture And Rural Development Of September 29, 2011 On The Scope And Method Of Keeping Medical And Veterinary Documentation And Records Of Animal Treatment And Templates Of This Documentation And Records (Journal Of Laws Of 2011, No. 224, Item 1347), Act Of December 18 2022 On Animal Medical Facilities (Journal Of Laws 2019.24).4. The Contractor Is Obliged To Execute The Order On The Terms And Conditions Described In The Draft Contract, Annex No. 6A, And The Description Of The Subject Of The Order, Which Constitutes Annex No. 9A To The Swz.
