Tenders Are Invited For Provision Of Care Services, Excluding Specialized Care Services For People With Mental Disorders In The Zabłudów Commune For 2026.
1. The Subject Of The Order Is The Provision Of Care Services At The Place Of Residence As A Form Of Generally Available Support In Everyday Functioning, Including For Older People And People With Disabilities. The Services Provided Will Be Tailored To The Needs Of Participants While Maintaining Equal Access To People Who, Due To Age, Health Condition Or Disability, Require Care Or Support Due To The Inability To Independently Perform Basic Everyday Activities; 2. Services Will Be Provided 5 Days A Week - From Monday To Friday From: 7:00 A.M. - 7:00 P.M. (Excluding Holidays, Saturdays And Sundays) And In Particularly Justified Cases, Services May Be Provided 12 Hours A Day By One Caregiver.3. Support For Care Services Will Cover 16 Residents Of The Municipal Social Welfare Center In Zabłudowo, Who Will Be Cared For By 4 People. 4. The Scope Of Care Services Includes In Particular: 1) Assistance In Meeting Everyday Life Needs, 2) Hygiene Care, 3) Care Recommended By A Doctor, Which Includes Care Activities Resulting From The Submitted Medical Certificate Or Medical Documentation, Supplementary To Community Nursing Care, 4) Ensuring Contacts With The Environment; 5) Services Will Be Provided With A Frequency Depending On The Needs Of The Person Receiving The Service, The Average Number Of Hours Of Care Services Provided Per Person Is 21 Hours Per Month;5. Care Services Are Provided On The Basis Of An Administrative Decision Specifying The Place Of Service Provision, Its Duration, Payment Rules, And The Scope Of Activities To Be Performed. 6. Care Services May Be Provided By Persons Who: 1) Have Qualifications To Perform One Of The Professions: Community Caregiver, Assistant To A Disabled Person, Nurse, Caregiver For The Elderly, Medical Caregiver, Qualified Caregiver In A Social Welfare Home Or 2) Have Experience In Providing Care Services, Including Professional, Voluntary Or Personal Experience Resulting From Performing The Role Of An Actual Caregiver And Have Completed Training In The Scope Of The Service Provided, Including First Aid Or Pre-Medical Assistance. 3) Has A Valid Certificate Issued By The National Court Register7. The Ordering Party Estimates That On Average 16 To 20 People Will Use The Services Per Month. The Estimated Number Of Hours For The Entire Order, I.E. Over A 12-Month Period, Is Set At 4,000 (Thousand) Hours. 8. Care Services Will Be Provided In The Zabłudów Commune In The Residents Apartments, To Which Caregivers Are Obliged To Reach By Their Own Transport. The Travel Costs Of The Caregiver Will Be Covered By The Contractor And Must Be Included In The Calculation Of The Hourly Rate For The Service Provided In The Offer. 9. The Unit Price For 1 Hour Of Providing Care Services At The Place Of Residence Includes All Costs Related To The Execution Of The Order And Should Also Take Into Account The Change In The Minimum Remuneration For Work, Which Will Apply From January 2026, The Costs Of Third Party Liability Insurance, Acc
