This Grant Application Is A Competitive Process Issued By The Ministry To Identify Organizations Interested In And Capable Of Delivering Capcc Initiative Activities, In Accordance With The Specifications, Terms, And Conditions Outlined In This Document.The Ministry Will Use This Transparent Process To Assess Applicants’ Qualifications, Organizational Capacity, Partnerships And Collaborations, Delivery Plan, And Budget. Grant Applicants Are Invited To Submit Their Applications In Line With All Specifications And Conditions Provided.Applicants Should Carefully Review All Requirements In This Grant Application. Failure To Meet Any Term, Condition, Or Requirement May Result In The Submission Being Rejected.1.2 Objectivesthe Community Access For People In Continuing Care (Capcc) Initiative Provides Funding To Agencies To Deliver Community Supports To Albertans With Disabilities. These Community Supports Address Social Isolation And Loneliness, Removing Barriers To Community Access And Social Inclusion For Adults Under The Age Of 65 Living In Continuing Care Homes. Using A Person-Centered And Flexible Approach, Community Supports Are Provided To Eligible Individuals To:•Promote Meaningful Participation In Community Activities;•Promote Opportunities For Social Inclusion; And•Develop, Maintain, And Enhance The Involvement Of Family And Friends.Individuals May Self-Refer Or Be Referred By A Variety Of Sources That Include, But Are Not Limited To, Continuing Care Staff, Family, Friends, And Community Grant Recipients.This Grant Application Describes How Capcc Funding Will Be Used To Align Service Activities With The Capcc Policy, And Core Elements Of The Initiative.
