The Subject Of The Order Is The Delivery Of Tools To A Hospital Clinic - A Surgical Clinic. A Detailed Description Of The Subject Of The Order Specifying The Product Range, The Ordering Partys Requirements And Quantities Is Included In Annex No. 2 To The Swz - Product And Price Form. By Completing The Requirements Specified In Annex No. 2 To The Swz, The Contractor Confirms That It Meets The Requirements Specified By The Ordering Party. The Method Of Implementing The Subject Of The Order Is Specified In The Draft Contract, Namely Annex No. 3 To The Swz. The Maximum Delivery Date Is 12 Weeks From The Date Of Signing The Contract (Taking Into Account The Provisions In Chapter Xviii Of The Swz - The Delivery Date Is One Of The Criteria For Evaluating The Offers). The Offered Contract Items Must Be Brand New, Complete, Of A High Standard Both In Terms Of Quality And Functionality, And Free From Material Defects, Constructional And Legal. The Offered Items Of The Order Must Be Approved For Trading And Use In Accordance With Applicable Law. In All Provisions Of The Swz And Its Annexes In Which The Ordering Party Refers To Standards, Approvals, Technical Specifications Or Reference Systems Or Indicates Trademarks, Parameters Or Sources Of Origin (Names Of Manufacturers Or Devices), In Accordance With Art. 99 Section 5 And Art. 101 Section 4 Of The Public Procurement Act, The Ordering Party Allows Equivalent Solutions. The Subject Of The Order Must Be Consistent With The Act Of April 7, 2022 On Medical Devices (Consolidated Text: Journal Of Laws 2022.974 Of 2022/05/09), If The Ordering Party Requires The Offer Of An Assortment Constituting A Medical Device. The Ordering Party Does Not Reserve The Possibility Of Applying For The Order Only By Contractors Referred To In Art. 94 Of The Public Procurement Law. The Ordering Party Does Not Impose The Requirements Referred To In Art. 96 Of The Public Procurement Law. The Contractor Is Obliged To Provide, Along With The Delivery Of The Tools, Complete Documentation, Including: A) A List Of Cleaning Agents Approved For Manual Cleaning Of Difficult Dirt On The Instruments (The Mentioned Agents Must Be Effective In Removing Difficult Dirt From The Tools, Without Damaging The Tools.) B) A List Of Cleaning Agents For The Maintenance Of The Tools Before Steam Sterilization, C) Other Documents Indicated In Annex No. 3 To The Swz.
