The Subject Of The Order Concerns Light Bulbs For Adults And Children, In Accordance With Administrative Decisions Issued By The Social Welfare Center In Kamieniec Ząbkowicki. The Provision Of Specialized Care Services For People With Mental Disorders In 2026 Will Be Based On The Provisions Of: A) Art. 8 And 9 Of The Act Of 19 August 1994 On Mental Health Protection (Journal Of Laws Of 2024, Item 91), B) Art. 18.1 Point 3 Of The Act Of March 12, 2004 On Social Assistance (Journal Of Laws Of 2025, Item 1214, As Amended), And In Accordance With The Scope Specified In: C) Regulation Of The Minister Of Social Policy Of September 22, 2005 On Specialized Care Services (Journal Of Laws Of 2024, Item 816).4. The Services Will Be Provided From The Date Of Conclusion Of The Contract To December 31, 20255. Estimated Average Number Of Service Recipients: 96. Estimated Number Of Hours Of Services During The Term Of The Contract: Approximately 3,540 Hours. The Number Of Hours And People Covered By The Services May Change And Cannot Be Determined Precisely Due To The Specific Nature Of The Order, Which Is Characterized By Variability Of Customer Needs Due To Changing Health Conditions. An Hour Of Service Is A Clock Hour (60 Minutes) Counted As Actual Time Worked, Without Traveling To Or From The Place Of Residence Of The Clients. 7. Services Should Be Provided At The Persons Place Of Residence. With The Consent Of The Childs Parent And In Consultation With The Ordering Party, Services May Be Provided Outside The Childs Place Of Residence, If This Results From The Childs Identified Needs. If The Services Are Provided Outside The Place Of Residence, They Should Be Provided By The Contractor In The Kamieniec Ząbkowicki Commune.8. The Gross Price Applies Per Hour.9. Specialized Care Services For People With Mental Disorders Will Be Provided By The Contractor In The Kamieniec Ząbkowicki Commune.
