1. The Subject Of The Order Is The Performance Of Equipment And Transport Works As Part Of The Ongoing Maintenance Of Roads In 2026 In The City And Commune Of Choroszcz. 2. Detailed Description Of The Subject Of The Order: - Work Consisting In Repairing The Surface Of Municipal Roads By Incorporating A Mixture Of Natural Aggregate Together With Embedding It In The Road Body As A Surface Layer Of Unbound Aggregate (Contractors Material) Together With Profiling And Compaction At The Place Of Installation - The Estimated Quantity Is 21,000 Tons. The Scope Of Work Includes: Obtaining The Aggregate Mixture, Loading, Transport To The Place Construction, Unloading, Spreading Of Material Using A Road Grader. The Ordering Party Requires That The Aggregate Be Delivered Directly From The Place Of Extraction To The Place Of Construction. The Ordering Party Requires That The Mixture Of Natural Aggregate Intended For The Execution Of The Order Meets The Requirements Specified In Annex No. 3 To The Order No. 102 Of The General Director For National Roads And Motorways Of November 19, 2010 Unbound Mixtures For National Roads Wt-4 2010 Technical Requirements In Terms Of Technical Requirements For Mixtures For Pavement Made Of Unbound Aggregate For Unbound Mix 0/31.5 For Pavement Made Of Unbound Aggregate: A) The Grain Size Of The Mixture Should Be Within The Area Of The Grain Limit Curves Shown In Figure No. 19 (Appendix No. 9 To The Swz), B) The Mixture Should Meet The Requirements Specified In Table 6 (Appendix No. 10 To The Swz) With The Exception Of The Requirements Removed From The Table, C) The Ordering Party Requires That The Mixture Natural Aggregate Came From A Mine With A Factory Production Control System, D) For Each Repair Carried Out, The Contractor Is Obliged To Provide Appropriate Documents (E.G. Consignment Notes, Wz Forms, Etc., Valid At The Contractors Plant And Agreed With The Ordering Party) Confirming The Amount Of The Delivered Aggregate Mixture Based On Weighing Carried Out Using A Legalized Scale And Appropriate Documents Confirming That The Aggregate Meets The Requirements Regarding Its Quality; The Documents Will Be Provided To The Ordering Party Each Time The Repair Is Completed, E) The Ordering Party Reserves The Right To Carry Out Control Tests Of The Delivered And Incorporated Aggregate Mixture In Order To Verify The Quantity And Quality Of The Delivered Aggregate (Control Weighing Before Placing The Aggregate, Laboratory Tests Of The Incorporated Mixture); The Subject Of The Tests Will Be Batches Of Aggregate Delivered Randomly Selected By The Ordering Party, F) If The Ordering Party Finds That The Quality Of The Delivered Aggregate Is Not Consistent With The Specified Requirements, The Contractor Will Cover The Cost Of Carrying Out Control Tests Ordered By The Ordering Party, While The Ordering Party Will Not Pay For The Delivered And Incorporated Aggregate Used To Repair The Entire Ordered Road Section, G) If The Ordering Party Fi
