1. The Subject Of The Order Is The Provision Of Personal Assistant Services To A Person With A Disability Who Is A Resident Of The Brudzień Duży Commune.2. The Services Of A Personal Assistant For A Person With A Disability Will Be Provided Under The Program Of The Minister Of Family, Labor And Social Policy Personal Assistant For A Person With A Disability For Local Government Units - 2026.3 Edition. The Expected Maximum Number Of Hours Of Services Provided By A Personal Assistant To A Person With A Disability Under The Order Is 7,886.50 Hours, Including: 1) 7,776 Hours For Persons With A Certificate Of Severe Disability, Including Multiple Disabilities (Including An Equivalent Certificate), 2) 110.5 Hours For Persons With A Certificate Of A Moderate Degree Of Disability (Including An Equivalent Certificate).4. The Expected Number Of Disabled People For Whom Personal Assistant Services Will Be Provided Under The Contract Is 31 People, Including: 1) 30 Disabled People With A Certificate Of Severe Disability, Including Multiple Disabilities (Including An Equivalent Certificate), 2) 1 Disabled Person With A Certificate Of Moderate Disability (Including An Equivalent Certificate).
