Ambulance Services - Providing Transport Of Patients In Need Of Dialysis Treatment From Their Homes To The Dialysis Treatment Clinic At The Alexandrovsk Ead Umbal And Back. Providing Transportation Of Patients In Need Of Dialysis Treatment From Their Homes To The Dialysis Treatment Clinic At The Aleksandrovska Ead Umbal And Vice Versa. The Performance Of The Service Includes The Transportation Of Dialysis Patients From Their Homes On The Territory Of The City Of Sofia To The Dialysis Treatment Clinic At The Alexandrovska Ead Umbal At The Address: City Of Sofia, St. G. Sofiyski St. No. 1 And Vice Versa. Transportation Will Be Carried Out Every Day Of The Week (Including Weekends And Holidays), Except For Sundays, Based On A Preliminary Monthly Schedule Presented By The Employer At The End Of Each Month For The Following Month, Which Includes: The Days And Hours Of Transportation, The Names, Addresses And Contact Telephone Numbers Of The Patients Who Will Be Transported. The Contractor Is Obliged To Strictly Follow His Presented Monthly Schedule, And The Patients Must Be Transported To The Dialysis Treatment Clinic A Maximum Of 30 /Thirty/ Minutes Before The Start Time For Dialysis, Respectively, The Transport To Their Homes Should Take Place No Later Than 30 Minutes After The End Time Of Dialysis. As Of The Date Of The Announcement Of The Procedure, The Number Of Patients Of The Dialysis Treatment Clinic Who Use The Hospitals Transport Is About 30: For The Days Monday-Wednesday-Friday - An Average Of 18, Who Will Be Dialyzed And Transported In Three Shifts. For The Days Tuesday-Thursday-Saturday - An Average Of 12, Which Will Be Dialyzed And Transported In Two Shifts. The Approximate Estimated Number Of Patient Transports In Both Directions For The Entire Contract Period Of 24 Months, Incl. Return Trip Is 6000. The Contracting Authority Reserves The Right To Include (Add) New Patients.
