Providing The Personal Assistant To A Person With A Disability Service For Local Government Units - 2026 Edition - 5,093 Hours (In Words: Five Thousand Ninety-Three). The Number Of Hours Of Personal Assistance Services Provided To A Given Person Will Be Indicated Each Time By The Social Services Center In Słupno. The Total Number Of Hours Provided During The Year May Be Increased By 10% Of The Order Value. Assistants Performing The Service Must Be Covered By Group Third Party Liability And Accident Insurance By The Contractor Throughout The Duration Of The Contract. The Ordering Party Will Refund The Costs Of Third Party Liability And Accident Insurance For Assistants Related To The Provision Of Personal Assistance Services In The Amount Of Up To Pln 200 Based On The Actual Costs Incurred After Presenting The Invoice. The Contractor Is Obliged To Direct 7 Assistants To Provide Personal Assistance Services For 13 People With Disabilities, With 1 Person For 1 Person With A Disability And 6 Assistants For 12 People With Disabilities. The Assistant Cannot Be A Person Who Is A Participant Of The Ministers Programs Financed From The Funds Resources Regarding Personal Assistance Services And Respite Care, Or A Person Who Is The Legal Guardian Of A Program Participant. When Implementing The Tasks, The Ordering Party Requires The Indication Of A Responsible Person, The So-Called Coordinator, Based On An Employment Contract Or A Civil Law Contract, Whose Scope Of Responsibility Includes Daily Planning, Coordination And Supervision Of The Work Of Caregivers Providing Services, Including Continuous Supervision Over The Implementation Of Services In The Field, Especially On Days Off And Weekends, In Particular In Emergency Situations, E.G. Problems With Getting The Caregiver To The Wards Apartment, Taking Appropriate Actions To Save The Health And Life Of The Ward, And, If Necessary, Notifying The Appropriate Services And The Ordering Party. Moreover, It Is The Responsibility Of The Person Will Be Responsible For: • Preparing A Monthly List Of Hours Of Services Provided, • Making Follow-Up Visits To Each Patient, At Least Once Every 3 Months, In Order To Identify, Among Others: Quality Of Service Provision, Proper Diagnosis Of The Needs Of The Ward, Depending On The Health Situation, Joint Planning Of Further Cooperation, Reporting To The Ordering Partys Employees Significant Observed Observations And Preparing Business Notes From These Visits, Which Will Be Kept In The Personal Files Of The Wards, • Immediate Organization Of Replacements And Providing Care For The Wards Due To The Caregivers Periodic Absence From Work, • Instructing The Caregivers In Detail On Their Scope Of Activities, Confirming This With An Appropriate Declaration Of The Performed Instruction, • Organizing Monthly Meetings Of All Caregivers At A Set Time In Order To Provide Service Implementation Cards For The Next Month, Discussing Important Matters Regarding The Provision Of Services To
