Cambridgeshire County Council Is Setting Up Access For 4 Analysts (Both Public Health Intelligence And Adult Social Care) To Our Population’S Linked Health, Care And Wider Determinants Anonymised Patient Level Data Held In The Dscro Environment For The First Time. We Want To Use This Linked Data Effectively To Produce Robust Analysis On The Patterns, Risks And Drivers Of Poor Health And Health Inequalities Within Our Population. We Require A Provider That Can Deliver Both Actionable Insight That Drives Improvement Work And Tackles Inequalities In Health, And Also Upskill The Capabilities Of Our Phi Analysts To Embed Population Health Management (Phm) Approaches In Our Work.The Council Is Looking To Contract A Provider To:1) Deliver Comprehensive Training For Analysts New To Using The Dscro Environment For Analysis Of Linked Health, Care And Wider Determinants Data. 2) Deliver Phi Insight Listed In The Scope Utilising Linked Health, Care And Wider Determinants Data Held In The Dscro Environment To Support A Population Health Management (Phm) Approach To Commissioning And Service Delivery Across Ccc. Work, Including Project Scope Specification, Is To Be Carried Out Collaboratively With Ccc Phi Analysts To Build Phm Skills And Capability Of Our Analysts.Outputs To Include Insight Products Covering: •Emergency Hospital Admissions For People Known To Adult Social Care, Including Falls•Mortality In People Known To Adult Social Care•Energy Performance Certificate (Epc) Data And Respiratory Disease Admissions
