3.1 The Subject Of The Order Is The Delivery Of An Ultrasound Device Along With Installation, Commissioning And Training Of Staff. 3.2 A Detailed Description Of The Subject Of The Order - Minimum Technical Requirements (Including Description Of Technical Parameters) Is Specified In Annex No. 6 To The Swz. 3.3 The Offered Subject Of The Order: 1) Must Be Brand New Or Post-Demonstration, Not Reconditioned, Complete, Free From Defects, Marked, And Have Operating Instructions In Polish; 2) Products Belonging To The Group Of Medical Devices Must Be Authorized For Marketing And Use In Medical Entities, In Accordance With The Act Of April 7, 2022 On Medical Devices (Consolidated Text: Journal Of Laws Of 2024, Item 1620) And Have Appropriate Certificates, Declarations Of Conformity Or Other Documents Required By Law. 3) Should Be Ready For Use Without The Need For Additional Purchases And Investments; 3.4 Year Of Production Of Medical Devices: Minimum 2024. 3.5 The Execution Of The Order Includes: A) Delivery Of Medical Devices To The District Medical Center In Wołów Spółka Z O. O., Al. Jerozolimskie 28, 56-120 Brzeg Dolny (Regional - Specialist Clinic) B) Installation, Assembly And Commissioning Of Medical Devices And Accessories, C) Conducting Training And On-The-Job Instruction In The Use Of The Medical Device And Staff Indicated By The District Medical Center In Wołów Spółka Z O. O. No Later Than The Date Of Delivery. 3.6 The Contractor Will Provide At Least A 24-Month Quality Warranty For The Supplied Medical Devices. The Contractor Will Also Provide A Warranty For The Materials, Devices And Equipment Used, For The Period Indicated Above, But Not Shorter Than The Manufacturers Warranty Period. All Warranty Cards And Conditions Will Be Provided To The Ordering Party Along With The Final Acceptance Reports. The Warranty Period Begins On The Day Following The Fault-Free Final Acceptance Of The Subject Of The Contract. During The Warranty Period, The Contractor Undertakes To Remove The Revealed Defects Free Of Charge Within 7 Days From The Date Of Reporting The Defect By The Ordering Party. If The Contractor Removes Significant Defects Under The Warranty, The Warranty Period Runs Anew From The Moment The Defect Is Removed. In Other Cases, The Warranty Period Is Extended By The Time During Which The Defect Was Removed. 3.7 The Warranty Period Runs Concurrently With The Warranty Period. Despite The Expiry Of The Guarantee Or Warranty, The Contractor Is Obliged To Remove Any Defects That Were Reported By The Ordering Party During The Guarantee Or Warranty Period. The Contractor Will Provide A Guarantee And Warranty For The Subject Matter Of The Contract Performed By Subcontractors Or Further Subcontractors. 3.8 Payment For The Delivered Subject Matter Of The Order Will Be Made In Four Monthly Installments (Maximum By March 31, 2026).
