The Subject Of The Order Is The Provision Of Exercise Therapy Services Using The Vojta Method For Four Children For A Total Of 32 Hours Per Month (Total Number Of Hours During The Implementation Of The Subject Of The Order: 384 Hours).2. The Subject Of The Order Is The Provision Of Specialized Care Services For People With Mental Disorders Referred To In § 2 Points 1, 3, 5 Of The Regulation Of The Minister Of Social Policy Of September 22, 2005. On Specialized Care Services (Journal Of Laws 2024, Item 816), I.E.: 1) Teaching And Developing Skills Necessary For Independent Living; 2) Physical Rehabilitation And Improvement Of Impaired Body Functions To The Extent Not Covered By The Provisions Of The Act Of August 27, 2004. On Health Care Services Financed From Public Funds;3) Ensuring That Children And Adolescents With Mental Disorders Have Access To Rehabilitation And Revalidation-Educational Classes, In Exceptional Cases, If They Are Unable To Obtain Access To The Classes Referred To In Art. 7 Of The Act Of 19 August 1994. On Mental Health Protection.3. The Order Must Be Performed In Accordance With: 1) Articles 3, 8 And 9 Of The Act Of August 19, 1994. On Mental Health Protection (I.E. Journal Of Laws Of 2024, Item 917).2) Art. 18 Section 1 Point 3 And Art. 50 Section 1, 2, 4, 7 Of The Act Of March 12, 2004. On Social Assistance (Journal Of Laws Of 2025, Item 1214).3) Regulation Of The Minister Of Social Policy Of September 22, 2005 On Specialized Care Services (Journal Of Laws Of 2024, Item 816).4. An Hour Of Service Is A Clock Hour (60 Minutes) Counted As The Actual Time Worked At The Clients Home, Excluding Travel Time Or Arrival Time To The Clients Place Of Residence.5. Personal Data Of The Charges Of The Municipal Social Welfare Center In Kowalewo Pomorski Directed To Provide Them With Specialized Care Services Will Be Transferred To The Contractor On The Basis Of A Personal Data Entrustment Agreement - In Accordance With Regulation (Eu) 2016/679 Of The European Parliament And Of The Council Of 27 April 2016 On The Protection Of Natural Persons With Regard To The Processing Of Personal Data And On The Free Movement Of Such Data And Repealing The Directive 95/46/Ec General Data Protection Regulation).6. The Ordering Party Requires That Caregivers Provide Services In The Places Of Residence Of People Using Specialized Care Services.7. To Provide Specialized Care Services For People With Mental Disorders, The Contractor Is Obliged To Employ People With No Criminal Record, Who Are Physically And Intellectually Fit, Able To Perform Manual Work, And Have The Ability To Maintain Proper Interpersonal Contacts. Note: The Ordering Party Reserves The Right To Reduce Or Increase The Indicated Amounts Of Services. The Ordering Party Will Only Pay For The Hours Of Services Actually Provided. The Number Of Hours May Change And Cannot Be Determined Precisely Due To The Specificity Of The Order, Which Is Characterized By Variability
