Providing Services Consisting In Providing A Temporary Place In A Shelter With Care Services For Homeless People Referred By The Municipal Social Welfare Center In Baranów In Accordance With The Provisions Of Art. 48A Of The Act On Social Assistance Of March 12, 2004. The Subject Of The Order Is To Provide 24-Hour Temporary Shelter With Full Board And The Provision Of Necessary Social Conditions And Care Services To Homeless Persons Referred By The Social Welfare Center Who, Due To Age, Illness Or Disability, Require Partial Care And Assistance In Meeting Their Necessary Life Needs. - Service Provision Period: From January 1, 2026. – December 31, 2026, 7 Days A Week. - The Contractor Is Obliged To Indicate In The Offer The Place Where The Service Will Be Provided. The Ordering Party Reserves The Right To Verify The Indicated Place In Terms Of Preparation For The Implementation Of The Ordered Service Before Concluding The Contract And Throughout The Entire Order Execution Period. - The Contractor Undertakes To Determine The Readiness For A Person Staying In A Shelter Who Leaves The Facility Within 24 Hours. During This Time, The Contractor Undertakes To Reduce The Costs Of Stay To An Amount Not Exceeding 50% Of The Amount Specified In The Contract. - The Contractor Undertakes To Determine Readiness For A Person Staying In A Shelter Who, Due To Medical Indications, Must Stay In A Medical Facility. During This Time, The Contractor Undertakes To Reduce The Costs Of Stay To An Amount Not Exceeding 50% Of The Amount Specified In The Contract. - The Contractor May Indicate In The Offer A Shelter Located A Maximum Of 200 Km From Baranów. - The Expected Number Of Homeless People Covered By The Service Is 2 People. Due To The Specificity Of The Order, Which Is Characterized By Variability Of Customer Needs Due To Their Changing Health Condition And Personal Situation, The Number Of Homeless People Provided By The Ordering Party Is The Number Expected During The Entire Duration Of This Order. The Ordering Party Reserves The Right To Reduce Or Increase The Above. Number Of People Depending On The Actual Needs In This Area. The Contractor Declares That He Accepts The Above Reservations And Will Not Be Entitled To Any Claims, Including Monetary Ones, Against The Ordering Party. - Admission Of A Homeless Person To The Shelter Will Take Place After Prior Arrangement Of The Date, Based On A Personal Referral Issued By The Municipal Social Welfare Center In Baranów. - The Contractor Is Obliged To Immediately Inform The Ordering Party About Any Change In The Life And Health Situation Of The Person Using The Shelter. - The Ordering Party Reserves That The Services May Only Be Provided By Entities Running A Shelter For Homeless People Entered By The Voivode On The List Of Establishments Providing Temporary Shelter. - The Standard Of Basic Services Provided In A Shelter For Homeless People And The Standard Of The Facility In Which The Shelter For Homeless People Is Lo
